Medical rehabilitation plan and process in a patient with cervical dystonia and established deep brain stimulation

Abstract:

Bakalářská práce se zabývá neurologickým onemocněním cervikální dystonií a je rozdělena do tří částí. První část zahrnuje informace, které se týkají obecně dystonie a jejího dělení. Dále je již zaměřená konkrétně na dystonii cervikální. Popisuje anatomii a fyziologii bazálních ganglií, epidemiologii, etiologii, patogenezi, klinický obraz, průběh onemocnění, diferenciální diagnostiku a léčbu cervikální dystonie. V léčbě je podrobněji popsána především terapie botulotoxinem a hluboká mozková stimulace. Ke konci první části je zmíněná generalizovaná dystonie. Druhá část je zaměřená na léčbu rehabilitační. Pojednává o přístupech, které je možné během rehabilitace využít. Zahrnuje kinezioterapii, speciální metodiky, fyzikální terapii, ergoterapii a psychologickou a sociální problematiku onemocnění. Poslední část bakalářské práce tvoří kazuistika, která je zaměřená na konkrétního pacienta s cervikální dystonií a zavedenou hlubokou mozkovou stimulací. Využívá teorii z první a druhé části a je uplatněná autorem v procesu léčebné rehabilitace. Obsahuje vstupní i výstupní vyšetření, popis konkrétních terapií a návrh dlouhodobého léčebně-rehabilitačního plánu.

Abstract:

This bachelor thesis discusses the neurogical illness cervical dystonia, and it is divided into three parts. The first part contains information regarding dystonia in general and its division. Then it focuses specifically on the cervical dystonia. It describes the anatomy and physiology of the basal ganglia, epidemiology, etiology, pathogenesis, clinical picture, the course of disease, differential diagnostics and the treatment of cervical dystonia. The treatment part describes primarily the botulotoxin therapy and deep brain stimulation. The end of the first part mentions generalized dystonia. The second part is focused on the rehabilitation therapy. It deals with approaches which are possible to use during the rehabilitation. It contains kinesiotherapy, special methodology, physical therapy, occupational therapy and psychological, as well as social aspect of the illness. The last part of the bachelor thesis consists of the case study, whose concern is a specific patient with cervical dystonia and established deep brain stimulation. It uses theory mentioned in the first and second part, and it is used by the author in the process of therapeutic rehabilitation. It contains the entrance and exit examination, the descriptions of specific therapies and proposal for long-term medical rehabilitation plan.
