Model of inpatient end of life care

Abstract:

Tato diplomová práce se věnuje problematice hodnocení zdravotní péče v závěru života pacientů s kolorektálním karcinomem. Závěr života představuje velkou zátěž pro pacienta i pro zdravotní systém vzhledem k náročné léčbě a souvisejícími náklady. K hodnocení lze využít trajektorie, které odrážejí léčebné modality, jež pacient v závěru života podstoupil. Cílem této práce je pomocí vhodných metodických nástrojů popsat a hodnotit péči v závěru života u pacientů s vybraným onemocněním v rámci dostupných dat. Teoretická část zahrnuje úvod do problematiky péče v závěru života a péče paliativní, je představeno onemocnění, které bude předmětem zájmu v praktické části – kolorektální karcinom. Následuje rešerše metod založených na analýze přežití vhodných pro hodnocení a modelování trajektorií pacienta. V praktické části je na reálných datech použita metodika vícestavových modelů. Výsledky naznačují, že v České republice je paliativní péče zahajována relativně pozdě a u menšího procenta pacientů, než tomu je v odborné literatuře.

Abstract:

This diploma thesis deals with the evaluation of health care provided at the end of life for patients with colorectal cancer. The end of life represents an enormous burden for the patient and the health system due to the demanding treatment and related costs. Trajectories reflecting the treatment modalities a patient underwent at the end of life can be used for its description and evaluation. The aim of this work is to use appropriate methodological tools to describe and evaluate end-of-life care for patients with selected disease within the available data. The theoretical part includes an introduction to the issues of end-of-life care and palliative care, a disease is presented that will be of interest in the practical part - colorectal cancer. The following is a search of survival analysis-based methods suitable for evaluating and modeling patient trajectories. In the practical part, the methodology of multistate models is used on real data. The results suggest that the initiation of palliative care is relatively late and in a smaller percentage of patients in the Czech Republic than in the literature.
