Awereness of Nosocomial Infections in Paramedics and Medical Students

Abstract:

I přes významný pokrok medicíny zůstávají nozokomiální nákazy (dále jen NN) stále významnou příčinou morbidity a mortality hospitalizovaných pacientů a jejich incidence nezaznamenává klesající trend. Velmi důležitá je dostatečná informovanost o této problematice, s důrazem na prevenci u zdravotníků a studentů připravujících se na budoucí povolání ve zdravotnictví. Znalost u obou skupin může přispět ke zlepšení kvality zdravotní péče a snížení incidence NN. Teoretická část práce je zaměřena na osvětlení pojmu nemocniční nákaza, charakteristiku procesu vzniku a šíření NN, nejdůležitější původce NN a nejčastější NN, na charakteristiku problematiky NN a základní možnosti potlačení NN, s důrazem na hygienu rukou. Cílem práce bylo zjistit úroveň informovanosti zdravotníků a studentů lékařské fakulty (dále jen studenti LF) o problematice nemocničních nákaz. Pro zjištění úrovně informovanosti byl zvolen dotazníkový průzkum u 160 zdravotníků ze čtyř nemocnic a 160 studentů Lékařské fakulty Ostravské univerzity v Ostravě. Z dotazníkového šetření vyplývá, že 88 % respondentů ví, co to jsou nemocniční nákazy. U obecných znalostí NN se lépe orientovali studenti LF, v odpovědích mezi skupinami zdravotníků a studentů LF však nebyl zjištěn statisticky významný rozdíl. V praktických znalostech zaměřených na hygienu rukou byli úspěšnější zdravotníci, v odpovědích mezi skupinami nebyl zjištěn rozdíl. Znalost MRSA, jako jednoho z nejvýznamnějších nozokomiálních problémů dnešní doby, je u obou skupin na dobré úrovni. Autoři uvádějí, že průměrná incidence se ve vyspělých zemích pohybuje mezi 6 ? 8 %, drtivá většina zdravotníků a studentů LF uvedla 1 ? 4 %. Podstatná část respondentů se domnívá, že zvýšený výskyt může ukazovat na nedbalost ve zdravotní péči. Jsou si správně vědomi skutečnosti, že nemocnice, která hlásí nulový výskyt, není výborná. 76 % zdravotníků se domnívá, že má dostatek informací o NN. Zdravotníci získávají největší množství informací ze seminářů a od svých nadřízených. Z výsledků vyplývá, že zdravotníci i studenti LF mají teoretické znalosti o problematice NN, ale je důležité u těchto skupin nadále zvyšovat povědomí o problematice, s důrazem na jejich prevenci.

Abstract:

Nosocomial infections (follow as NI) remain significant cause of morbidity and mortality in hospitalized patients and the incidence is not decreasing, despite of significant progress in medicine. Sufficient awareness is very important about this issue with emphasis in prevention in paramedics and students preparing for future career in health care. Knowledge of both groups can contribute to improve quality of health care and to decrease the incidence of hospital infections. Theoretical part is focused on explanation of hospital infection concept, process of emergence and spreading of NI, the most important originator and the most common NI, on characteristics and problematics of NI and possibilities in suppression of basic NI, with emphasis on the hand hygiene. Level of knowledge in paramedics and medical students about awareness of the hospital infections was the aim of this thesis. A group of 160 health professionals from four hospitals and 160 medical students of the University of Ostrava were chosen in the questionnaire survey to find level of awareness. The questionnaire survey shows that 88 % of respondents know what NI means. Medical students proved better orientation in general knowledge of NI, but there was no difference in the responses between groups of paramedics and medical students. The paramedics were more successful in the practical knowledge focused on the hand hygiene, but there was no difference in the responses between both groups. Knowledge of MRSA, as one of the most important nosocomial problem nowadays, is on higher level in both groups. The average incidence in developed countries is in range 6 - 8 % as the authors suggest, but the vast majority of paramedics and medical students have indicated 1 - 4 %. A significant proportion of respondents believe that the increased incidence of NI may indicate negligence in health care. They are well aware in fact that hospitals which report zero incidence is not excellent. 76 % of paramedics think, that they have enough information about NI. The most information paramedics acquire from seminars and from their supervisors. The results show that paramedics and medical students have a theoretical knowledge about NI, but it is important for these groups continously raise awareness in issues with emphasis in prevention.
