Nursing Care Plan for Patients with Pressure Ulcers

Abstract:

Existuje nepřeberné množství definic dekubitů. V drtivé většině je v etiologii zmiňována základní entita ? tlak. Dekubitus je mezinárodní definicí dekubitů podle EPUAP (European Pressure Ulcer Advisory Panel), NPUAP (National Pressure Ulcer Advisory Panel) a PPPIA (Pan Pacific Pressure Injury Alliance) (2014, s. 12) popisován jako lokalizované zranění kůže a/nebo podkožní tkáně obvykle nad kostní prominencí jako důsledek tlaku nebo tlaku v kombinaci se střižnou silou. Teoretická část práce má dvě kapitoly. První kapitola je orientována na vymezení pojmů dekubitus. Jsou zde uvedeny poznatky o lokalizaci, mechanismu vzniku, rizikových faktorech, klinických stádiích, léčbě a komplikacích. Druhá kapitola zahrnuje informace o intervencích, které sestra v péči o pacienty s dekubity uplatňuje, a o ošetřovatelských preventivních strategiích vzniku dekubitů, jejichž implementace je prvním předpokladem správné léčby. V empirické části práce je vypracován vzorový plán ošetřovatelské péče o pacienta s dekubitem, včetně ošetřovatelské diagnostiky, cílů a intervencí. Cílem bakalářské práce je koncipovat vzorový obecný plán ošetřovatelské péče o pacienty s dekubity a charakterizovat specifika ošetřovatelské péče o tyto pacienty. Dalším cílem je stanovit relevantní ošetřovatelské diagnózy dle NANDA International taxonomie II, cíle ošetřovatelské péče a navrhnout ošetřovatelské intervence.

Abstract:

There is an infinite number of definitions of the pressure ulcers. A fundamental entity ? pressure is predominantly mentioned in etiology. A pressure ulcer is an international definition by EPUAP (European Pressure Ulcer Advisory Panel), NPUAP (National Pressure Ulcer Advisory Panel) and PPPIA (Pan Pacific Pressure Injury Aliance) and it is described as localized injury to the skin and/or underlying tissue usually over a bony prominence, as a result of pressure, or pressure in combination with shear. The theoretical part of my thesis has two chapters. The first chapter refers to specification of the terms of the pressure ulcer. There are the findings of localization, origin mechanism, the risk factors, the clinical stages, medical treatment and the complications. The second chapter collects the information about interventions which a nurse uses when she takes care of patients with pressure ulcers and about nursing preventive strategies of the pressure ulcers origin whose implementation is considered to be a first assumption of correct medical treatment. The sample plan of the nursing care of the patient with pressure ulcer is elaborated in the empirical part of my thesis including nursing diagnosis, the aims and the interventions. The aim of my bachelor thesis is to design a universal sample plan of the nursing care of the patients with pressure ulcers and define the specifics of the nursing care of those patients. The follow-up aim is to establish relevant nursing diagnosis by NANDA International taxonomy II, set targets of nursing care and devise nursing interventions.
