Management of nursing care for patients with artificial lung ventilation in home care

Abstract:

Diplomová práce "Management péče sester o pacienty s umělou plicní ventilací v domácí péči" se věnuje aktuálnímu problému umělé plicní ventilace v domácím prostředí, tentokráte z pohledu zdravotních sester domácí péče. V teoretické části je práce zaměřena na vysvětlení základních pojmů, jenž se dotýkají managementu péče o pacienta s domácím plicním ventilátorem. Tato část práce je čerpána především z poznatků jiných autorů. Osvětluje, co znamená pojem management v ošetřovatelství, co je to umělá plicní ventilace, jaká je vlastní ošetřovatelská péče související s touto zdravotní zátěží, dále pak vysvětluje, co je to domácí umělá plicní ventilace i vlastní systém domácí péče. V empirické části je práce věnována zmapování znalostí sester domácí zdravotní péče v oblasti domácí umělé plicní ventilace, zjištění jejich způsobů a zdrojů vzdělávání. A v neposlední řadě i zjištění, jaké jsou jejich kompetence při péči o pacienty závislé na domácí umělé plicní ventilaci a zda jsou s nimi spokojeni. K dosažení těchto výsledků bylo použito dotazníkové šetření, jenž bylo poskytnuto sestrám z agentur domácí péče, které se již starají o pacienty s domácí umělou plicní ventilací. Celkově se dotazníkového šetření zúčastnilo 118 respondentů. Šetřením bylo zjištěno, že povědomost sester z agentur domácí péče v péči o intenzivního pacienta je nedostatečná, s tímto problémem se často nesetkávají. Rovněž je minimální nabídka specializačního školení, jež by sestrám v agenturách domácí péče napomohla při této vysoce erudované a specializované činnosti. Taktéž bylo šetřením zjištěno, že sestry z agentur domácí péče jsou vcelku spokojené s náplní své činnosti, zvýšení kompetencí by vyžadovalo pouze menší procento respondentů. Na základě zjištěných výsledků byl vytvořen návrh edukační brožury pro sestry z agentur domácí péče, která by měla zjednodušit orientaci v uvedené nelehké problematice.

Abstract:

Thesis: ´´management of nurses´ care of patients with artificial pulmonary ventilation in home care´´ is dedicated to actual problem of artificial pulmonary ventilation in home care, this time from the point of view of nurses. Theoretical part of the thesis is focused on the explanation of basic terms that are related to care management of patients with home artificial pulmonary ventilation. This part is borrowed from other authors´ works. It explains what is the meaning of the term ´´management´´ in nursing, it also explains what is artificial pulmonary ventilation, what means nurse caring itself related to this medical burden, further it is focused on what is home artificial pulmonary ventilation and how works the home caring system itself. The empirical part of the thesis is dedicated to the analysis of the knowledge of nurses working in home care in the field of artificial pulmonary ventilation, it is also focused on the modality and the sources of their education. Last but not least this thesis focuses on nurses´ expertise in caring about patients addicted to home artificial pulmonary ventilation and about nurses´s satisfaction with it. To achieve these results it was used questionnaires provided to nurses from home nursing agencies, that already have in their care patients on artificial pulmonary ventilation. 118 respondents participated on questionnaire. With this examination it was discovered that the knowledge of nurses from home caring agencies in intensive care is not sufficient, and that these nurses doesn't have much experience with this issue. At the same time the offer of specialised education trainings are minimal. These trainings would help nurses in this high specialized activity. It was also discovered that nurses from home caring agencies are satisfied with the scope of employment, enhancing expertises would be required from a lower percent of respondents. On the base of these findings it was created a proposal of educational materials for nurses employed in agencies for home caring. This educational materials should simplify the understanding of this complicated issue
