Dysphagia by Patient with Ischemic Stroke

Abstract:

Diplomová práce se orientuje na problematiku poruchy polykání spojené s akutní ischemickou cévní mozkovou příhodou. Výzkumu předcházející teoretická část práce se ve své první kapitole zaměřila na ischemický iktus s důrazem na terapii a příčiny vzniku, druhá polovina teorie poukazuje na problematiku poruchy polykání spojené s iktem. Zisk dat probíhal formou kvantitativního výzkumu za pomoci retrospektivní studie archivované dokumentace pacientů hospitalizovaných na neurologické klinice v roce 2020. Prvním cílem práce bylo zjistit, jaké faktory mají u ischemického iktu vliv na poruchu polykání hodnocenou screeningovým vyšetřením GUSS. Pohlaví a věk jako ovlivňující faktor potvrzeny nebyly, naopak výrazný vliv na stupeň dysfagie byl spojen s typem terapie a úspěšností rekanalizace mozkové tepny po mechanické trombektomii. Druhý cíl se díval na problematiku z druhé strany, a to konkrétně na výsledný počet bodů v GUSS a jeho vliv na provedení dalších intervencí. Zde byla potvrzena souvislost mezi výsledkem GUSS a vyšetřením pacienta objektivními metodami, změnami dietních opatření a provedením rescreeningu.

Abstract:

The diploma thesis focuses on the issue of swallowing disorders associated with acute ischemic stroke. The theoretical part of the research preceding the research focused on its first chapter on ischemic stroke, emphasising therapy and causes. The second half of the theory points to swallowing disorders associated with stroke. Data collection took place in the form of quantitative research with the help of a retrospective study of archived documentation of patients hospitalized at a neurology clinic in 2020. The first aim was to determine what factors affect ischemic stroke gender and age were not confirmed as influencing factors; on the contrary, a significant effect on the degree of dysphagia was associated with the type of therapy and the success of cerebral artery recanalization after mechanical thrombectomy. The second goal looked at the issue from the other side, namely the resulting number of points in GUSS and its impact on the implementation of other interventions. Here, the connection between the result of GUSS and the examination of the patient by objective methods, changes in dietary measures and rescreening was confirmed.
