Abstract:

Tato práce je zaměřena zejména na metody invazivní prenatální diagnostiky a srovnávání jejich nálezů chromozomových aberací. V teoretické části se zabývám popisem laboratorních metod, formulováním výhod a nevýhod. Dále popisuji indikace ke genetickému vyšetření, způsoby získávání materiálu, edukace matky ohledně vyšetření. Dále zmiňuji, co je prenatální diagnostika, přínos pro matku a plod, kam se historicky její metody posunuly. Teoretická část je členěna na dvě poloviny. V první se zabývám metodami klasické cytogenetiky. Druhá část obsahuje metody molekulární cytogenetiky. Praktická část práce srovnává metodu Array-CGH a karyotyp. Nutnost vyšetření karyotypu po vyšetření pomocí Array-CGH. Taktéž kolik nálezů balancovaných chromozomových aberací není teoreticky detekováno kvůli vyšetření pouze Array-CGH. Hodnotí také nálezy danými metodami. Zabývá se také otázkou zvýšení záchytu po zavedení Array-CGH namísto vyšetření karyotypu. Hodnocená data byla získána na pracovištích Genetika Plzeň a West v letech 2019-2023 od žen postupující prenatální genetické vyšetření.

Abstract:

This thesis focuses mainly on methods of invasive prenatal diagnosis and comparison of their findings of chromosomal aberrations. In the theoretical part I describe the laboratory methods, formulate advantages and disadvantages. Furthermore, I describe indications for genetic testing, methods of obtaining material, and maternal education regarding testing. I also mention what prenatal diagnostics is, the benefits for the mother and the fetus, where its methods have historically moved. The theoretical part is divided into two halves. In the first one I deal with the methods of classical cytogenetics. The second part contains methods of molecular cytogenetics. The practical part of the thesis compares the Array-CGH and karyotype methods. Necessity of karyotype examination after Array-CGH. Also how many findings of balanced chromosome aberrations are theoretically not detected due to Array-CGH examination only. It also evaluates the findings by the given methods. It also discusses the issue of increased detection following the introduction of Array-CGH instead of karyotype testing. The data evaluated were obtained at Genetics Pilsen and West between 2019 and 2023 from women undergoing prenatal genetic testing.
