Nursing Process in a Patient with Stoma

Abstract:

Bakalářská práce nese název Ošetřovatelský proces u pacienta se stomií. Teoretická část je zaměřena na problematiku kolorektálního karcinomu a jeho chirurgické řešení s vyvedením stomie. Dále je zde kapitola věnovaná problematice stomií, kde jsou rozděleny jednotlivé typy stomií v břišní chirurgii a jejich možné komplikace. Třetí kapitolou teoretické části je věnována specifikům ošetřovatelské péče o pacienty se stomií. V této kapitole jsou popsána specifika předoperační, perioperační a pooperační péče a možné komplikace u pooperačních stavů. Předoperační péče se věnuje přípravě pacientů u plánovaných i u akutních výkonů. V pooperační ošetřovatelské péči jsou popsány možné komplikace, kterým je třeba předcházet a při jejich vzniku je včas rozpoznat a rychle začít jednat. V této kapitole jsou zmíněna i sdružení, která spojují lidé s tímto onemocněním. V poslední kapitole teoretické části je uveden ošetřovatelský proces a jeho krátká charakteristika. V praktické části je vypracován ošetřovatelský proces pacientky s nově diagnostikovaným kolorektálním karcinomem. Nejprve byl zhodnocen celkový stav pacientky, na základě kterého byly stanoveny ošetřovatelské diagnózy a cíle. Dále byly stanoveny intervence potřebné ke splnění stanovených cílů, které byly následně realizovány. V závěru praktické části jsou uvedena doporučení pro praxi.

Abstract:

This bachelor essay is named The Nursing process in a patient with stoma. The theoretic part is focused on the issue of colorectal carcinoma and its surgical treatment by stoma. Next, there is a chapter focused on the issue of stoma, its individual types and their possible complications. The third chapter of the theoretic part deals with the specifics of nursing care for a patient with stoma. This chapter describes the specifics of preoperative, perioperative, and post-operative care, and the possible complications in post-operative states. Preoperative care is focused on preparing the patients for planned and acute treatments. Post-operative nursing care deals with possible complications to be prevented and in the case of their occurrence quickly discovered and swiftly treated. This chapter also mentions organisations that connect people with this disease. The last chapter of the theoretic part focuses on the nursing process and its brief description. The practical part follows the nursing process of a patient with newly diagnosed colorectal carcinoma. Firstly the overall state of the patient was assessed on the basis of which the diagnosis and the treatment goals were determined. The interventions needed to reach these goals were also determined. The conclusion of the practical part of the essay includes recommendations for medical practise.
