Subjective experience of disease in patients with multiple sclerosis

Abstract:

Cíl: Cílem této práce je zjistit subjektivní prožívání nemoci u pacientů s diagnózou roztroušená skleróza a zachytit jejich žitou zkušenost. Metodika: Pro dosažení cíle práce byla zvolena metoda kvalitativního přístupu, a to konkrétně interpretativní fenomenologické analýzy (IPA). Sběr dat byl realizován prostřednictvím individuálních polostrukturovaných rozhovorů s pěti záměrně vybranými participanty. Audionahrávky byly následně doslovně přepsány do textové podoby. Analýza textu byla provedena dle obecného analytického postupu IPA v kombinaci s metodou barvení textu. Výsledky: Ve vyprávěních participantů byly identifikovány tři tematické kategorie, které ilustrují biologické, psychologické a sociální aspekty prožívání u pacientů s roztroušenou sklerózou: Vstup roztroušené sklerózy do života, Náročné aspekty života s roztroušenou sklerózou, Adaptace na život s roztroušenou sklerózou. První kategorie rozkrývá prožívání konfrontace s onemocněním, v druhé kategorii jsou popsány náročné aspekty života s tímto onemocněním a třetí kategorie reprezentuje hledání způsobů, jak znovu získat kontrolu nad svým životem. Závěr: Práce zachycuje žitou zkušenost pacientů s roztroušenou sklerózou, a poskytuje tak kontext pro pochopení komplexity tohoto onemocnění. Vybraní participanti hovořili o prožívání období kolem onemocnění touto chorobou, o prožívání změn, které jim nemoc přinesla do života a o náročných aspektech každodenního života s nemocí. Pro nalezení nové rovnováhy v životě se jevila jako zásadní změna životního stylu a změna přístupu k nemoci. Důležitou roli hrála také podpora blízkých, přátel, RS komunity a empatické pracovní zázemí. Vzhledem ke kvalitativní povaze práce se výsledky nedají zobecnit.

Abstract:

Aim: The aim of this thesis is to ascertain the subjective experience of patients with multiple sclerosis (MS) and to capture their lived experience. Methodology: To achieve the aim of the thesis the method of qualitative approach was chosen, namely interpretive phenomenological analysis (IPA). Data collection was carried out through the individual semi-structured interviews with five deliberately selected participants. The audio recordings were transcribed verbatim. The text analysis followed the general IPA steps in combination with text highlighting technique. Results: Three thematic categories were identified from patients' accounts of their lives with MS: Entry of multiple sclerosis into their life, Challenging aspects of life with multiple sclerosis, Adapting to multiple sclerosis. The themes illustrate the biological, psychological and social aspects of experience in patients with multiple sclerosis. The first category reveals the experience of confrontation with the disease, the second category describes the challenging aspects of living with the disease and the third category represents searching for the ways to regain control of their lives. Conclusion: The thesis captures the lived experience of patients with multiple sclerosis and thus provides a context for understanding the complexity of the disease. Selected participants talked about experiencing the time prior to a diagnosis of MS and the time following the diagnosis, about experiencing the changes that the disease has brought to their lives and about the challenging aspects of everyday life with the disease. The results show that the disease affects many areas of life radically. Changing the lifestyle and participants' approach to their disease seemed to be fundamental to find a new balance in their life. The family and friends support, MS community support and having a supportive work environment was also important. Given the qualitative nature of this study, the results cannot be generalized.
