Client views of premature termination of therapy

Abstract:

Práce se zabývá zkušeností klientů s předčasným ukončením psychoterapie. Teoretický úvod objasňuje vymezení předčasného ukončení psychoterapie a shrnuje dosavadní vědecká zjištění. Výzkumy zaměřující se na předčasné ukončení terapie jsou většinou kvantitativního charakteru a zkoumají především jeho důvody a prediktory. V předložené studii byly provedeny polostrukturované rozhovory s osmi respondenty. Na základě interpretativní fenomenologické analýzy bylo identifikováno sedm nadřazených témata: „bezmoc klientů“, „terapie neřeší hlavní problém“, „terapeutický vztah“, „rigidnost postupů terapeuta“, „důvody odkládání ukončení terapie“, „impulsy k ukončení terapie“ a „hledání příčin nezdařilé terapie.“ Studie zjistila, že bez řešení hlavního problému nemá psychoterapie pro klienty význam. Pro respondenty bylo důležité rozumět tomu, jestli a jakým způsobem se k řešení problému směřuje. K neřešení problému přispívá nedostatečná dohoda na cílech terapie, nedostatečný individuální přístup terapeutů, nezájem terapeutů, nelepšící se stav klientů nebo neporozumění intervencím terapeuta. I přes pochybnosti o terapii se klienti před definitivním rozhodnutím ji ukončit dostavili na několik dalších sezení. Identifikovali jsme způsoby, kterými se klienti ve své zkušenosti pokoušeli oddálit odchod z terapie: hledání pozitivních charakteristik na terapeutovi, relativizace jejich pocitů a víra v to, že jim terapie přinese změnu. Všechny respondenty vedlo k odchodu z psychoterapie postupné přibývání věcí, se kterými nebyly spokojeni. Polovině respondentů pomohla k ukončení terapie významná událost.

Abstract:

The presented study is focused on the experience of clients who unilaterally discontinued psychotherapy. Theoretical introduction clarifies the definition of a premature termination of psychotherapy, including current research findings. The research ─focused mainly on the premature termination of psychotherapy─ is mostly quantitative in nature and examines primarily the reasons and predictors of it. In this study, semi-structured interviews were carried out with eight individuals who unilaterally discontinued psychotherapy. The data was analyzed using interpretative phenomenological analysis, which carried out seven topics presented beneath the headings: “helplessness“, “therapy does not solve the main problem”, “therapeutic relationship“, “rigidity of therapist’s procedures“, “reasons for delaying completion of the therapy”, “impulses for therapy termination” and “seeking the causes of failed therapies.“ If the therapy does not contribute to achieve the patient’s main treatment goals then the psychotherapy is considered as meaningless. For the respondents it’s important to understand whether and how their problems can be solved. Results suggest that the problem is unsolved when there are not enough agreements on the therapy goals, when therapist‘s non-individual approach or disinterest exist, when there are no improvement on the clients, or when there is some misunderstanded intervention from the therapist. It has been identified the ways that clients in their experience trying to delay the termination: finding positive therapist characteristics, relativizing their feelings and expecting that their therapy will bring them a change. All the respondents led to a termination of psychotherapy because of an upsurge of dissatisfaction. Half of the respondents terminated the therapy after a significant event.
