Analysis of ATM gene mutations and their functional impact in chronic lymphotic leukemia patient

Abstract:

Abnormality genu ATM reprezentují důležitý prognostický faktor u chronické lymfocytární leukémie (CLL). Samotná delece 11q (11q-) nevede k inaktivaci proteinu ATM, na rozdíl od bialelických defektů zahrnujících mutaci(e). V této práci jsme zkoumali mutace ATM u převážně vysoce rizikových pacientů za použití několika metod zahrnujících resekvenační čip, western blot a funkční test. Nově navržený funkční test je založen na sledování indukce genu p21 po paralelním ošetření CLL buněk fludarabinem a doxorubicinem, kdy dysfunkce ATM narušuje indukci p21 po doxorubicinu. Mutace ATM byla detekována u 16 % (22/140) pacientů, přičemž jejich frekvence byla nejvyšší u skupiny s 11q- (34 %). Všechny mutované vzorky vykazovaly dysfunkci ATM prokázanou funkčním testem a/nebo nulovou hladinou proteinu. Tato ztráta funkce ATM byla evidentní také skrze defektní aktivaci p53 dráhy po expozici buněk ionizujícímu záření. Buňky nesoucí mutaci ATM byly jasně rezistentní na doxorubicin, ale nikoli na fludarabin. Pacienti s mutací ATM vykazovali téměř identický čas do první terapie jako pacienti se samotnou 11q-. Celkově naše data ukazují, že mutace genu ATM vyskytující se samostatně nebo v kombinaci s 11q- vedou k prokazatelné dysfunkci ATM u pacientů s CLL a jejich přítomnost lze predikovat pomocí nového funkčního testu. Tato práce je komentovaným souborem publikací doktorandky.

Abstract:

ATM abnormalities represent important prognostic factor in chronic lymphocytic leukemia (CLL). Sole 11q deletion (11q-) does not result in ATM inactivation by contrast to biallelic defects involving mutation(s). In this thesis, we analyzed ATM mutations in predominantly high-risk patients using several methods including resequencing microarray, western blot and functional test. Newly designed functional test is based on p21 gene induction after parallel treatment of CLL cells with fludarabine and doxorubicin, where ATM dysfunction impairs p21 induction after doxorubicin. We detected ATM mutation(s) in 16 % (22/140) of patients with the highest frequency in group harboring 11q- (34 %). All mutated samples manifested ATM defect demonstrable by functional test and/or null protein level. The loss of ATM function was also evident through defective p53 pathway activation after ionizing radiation exposure. Cells with ATM mutation were obviously resistant to doxorubicin but not to fludarabine. Patients with ATM mutation(s) experienced nearly identical time to first treatment as those with sole 11q-. Altogether, ATM mutations either alone or in combination with 11q- led to demonstrable ATM dysfunction in patients with CLL and mutation presence can be predicted by innovative functional test.
