Nursing Process in Care of Patient with Morbus Crohn

Abstract:

Cílem bakalářské práce je sestavení názorného plánu ošetřovatelské péče u vybraného pacienta s Crohnovou chorobou. Plán ošetřovatelské péče bude vycházet z ošetřovatelských diagnóz NANDA International taxonomie II, které se vztahují k potřebám pacienta s touto chorobou. Informace pro zpracování praktické části bakalářské práce byla sbírána pomocí cíleného rozhovoru s pacientem dle struktur NANDA - International domén. Hlavním zdrojem informací byl samotný pacient a jeho ochota zodpovídat na dotazy. Dalším důležitým zdrojem byla zdravotnická dokumentace a nemocniční personál, který o pacienta pečoval. Během hospitalizace bylo na chirurgickém oddělení D stanoveno devět ošetřovatelských diagnóz podle NANDA International Taxonomie II. V ošetřovatelském procesu byly stanoveny cíle, intervence s následnou realizací a hodnocením. U většiny diagnóz byly splněny stanovené cíle ošetřovatelské péče. Pouze u diagnóz snaha zlepšit výživu a průjem došlo k částečnému splnění stanovených cílů. Pacient byl po šestidenní hospitalizaci na standardním oddělení chirurgie D propuštěn do domácí péče.

Abstract:

The aim of this bachelor thesis is to propose plan of nursing care of selected patient with Crohn disease. Plan of nursing care will be based on nursing diagnosis NANDA International Taxonomy II, which relate to the needs of patient with this disease. The information for processing the practical part of the bachelor thesis was collected by a targeted interview with the patient according to NANDA - International domains. The main source of information was patient and his willingness to answer questions. Another important source was the medical documentation and the hospital staff who took care of the patient. Nine nursing diagnosis were established according to NANDA International Taxonomy II during the hospitalization at the surgery department D. There were goals, interventions with consequent realization and evaluation set in the nursing process. For most diagnosis, the established goals of nursing care have been met. Only in diagnosis the effort to improve nutrition and diarrhea has partially met the set goals. Patient was discharged to home care after six days of hospitalization at the standard department of surgery D.
