Nursing Process in Care of a Child with Autism Spectrum Disorder

Abstract:

Předložená bakalářská práce je zpracována s cílem vytvořit názorný ošetřovatelský proces u dítěte s poruchou autistického spektra, které bylo hospitalizováno na dětském oddělení psychiatrické nemocnice. Bakalářská práce je rozdělena na část teoretickou a rozsáhlejší část praktickou. Teoretická část bakalářské práce zahrnuje dvě kapitoly. První kapitola s názvem Porucha autistického spektra poskytuje základní poznatky o daném onemocnění. Opomenuta není ani diagnostika, která je v léčbě daného onemocnění velmi podstatná. V podkapitole diagnostiky jsou zpracovány screeningové a diagnostické nástroje, na které můžeme při péči o dítě narazit. Druhá kapitola s názvem Problematická péče o dítě s poruchou autistického spektra pojednává o specifických potřebách a odlišném myšlení dětí s daným onemocněním, zmíněna je i role sestry při ošetřovatelské péči. Praktická část zpracovává ošetřovatelský proces u chlapce s vybranou formou poruchy autistického spektra. V tomto případě se jedná o třináctiletého chlapce s atypickým autismem, který byl přijat pro zhoršení stavu, zejména z důvodu agresivního chování. Při zpracování získaných dat prostřednictvím anamnézy bylo stanoveno devět aktuálních a tři potenciální ošetřovatelské diagnózy. U každé stanovené diagnózy jsme si stanovili reálně dosažitelný cíl s ohledem na přibližnou dobu hospitalizace. U každé diagnózy jsme zaznamenali intervence, jejich realizaci a následné vyhodnocení. Celkově bylo stanoveno 12 ošetřovatelských diagnóz. U 10 diagnóz byl cíl splněn. U dvou diagnóz byl cíl splněn částečně. Celková doba hospitalizace byla 47 dní, což je necelých sedm týdnů. Průměrná doba hospitalizace na dětském psychiatrickém oddělení je šest až osm týdnů.

Abstract:

The submitted thesis is made with objective to create an illustrative nursing process at a child with autism spectrum disorder which was hospitalized at the child department of a psychiatric hospital. The thesis is divided into the theoretical part and more extensive practical part. The theoretical part of the thesis includes two chapters. The first chapter with the name Autism Spectrum Disorder provides the basic facts about the given illness. Also mentioned is also diagnostic which is very substantial in treatment of the given illness. In the diagnostic subchapter are processed screening and diagnostic tools which we can encounter during the care for the child. The second chapter with the name Problematic Care for a Child with Autism Spectrum Disorder discusses about the specific needs and different thinking of children with the given disease, role of a nurse in the nursing care is also mentioned. The practical part analyzes the nursing process at a boy with a chosen form of autism spectrum disorder. In this case it is 13 years old boy with atypical autism who was accepted because his condition deteriorated, mostly because of aggressive behavior. To analyze the data gathered using anamnesis, nine current and three potential nursing diagnoses were determined. At every determined diagnosis, we also set a realistically achievable goal regarding the approximate length of the hospital stay. At every diagnosis we also noted interventions, their realization and following evaluation. Totally has been determined 12 nursing diagnoses. At 10 of nursing diagnoses the objective has been achieved. At two the objective has been partially achieved. The total length of hospital stay was 47 days, meaning less than seven weeks. The average length of the hospital stay at a child psychiatric ward is between six and eight weeks.
