Modern pattern recognition methods for image data analysis in neuropsychiatric research

Abstract:

Úvod. Diagnostika mentálních onemocnění je i v dnešní době značně subjektivní a občas ne zcela přesná, protože je stále založena především na pozorování a klinických rozhovorech. Moderní automatizované klasifikační metody využívající obrazy mozku, kognitivní či genetická data by mohly mít potenciál diagnostiku zrychlit a zpřesnit, problémem však je, že tato data jsou velice rozsáhlá, což vede k nestabilním výsledkům a zpravidla nízké klasifikační úspěšnosti. Z těchto důvodů klasifikaci často předchází redukce dat. Běžně používané redukční metody však mají různé nedostatky, proto bylo cílem této práce navrhnout plně automatizovanou vícerozměrnou redukční metodu, která by umožnila efektivní redukci dat vedoucí k úspěšnému rozpoznání pacientů s mentální poruchou od zdravých subjektů. Metody. V práci byly navrženy dvě metody, a to penalizovaná lineární diskriminační analýza s resubstitucí a penalizovaná lineární regrese s resubstitucí. Obě metody byly implementovány v prostředí MATLAB a zahrnuty do balíku s názvem ‘Penalised Reduction & Classification Toolbox’. Metody byly testovány na simulovaných i reálných datech pacientů se schizofrenií, Alzheimerovou chorobou i na datech zdravých dobrovolníků ve čtyřech experimentech. Výsledky. Navržené metody umožnily výběr oblastí mozku relevantních ke studovaným onemocněním ve všech experimentech. Klasifikace simulovaných dat redukovaných těmito metodami dosahovala úspěšnosti téměř 100 %. Výsledky rozpoznávání pacientů s Alzheimerovou chorobou, mírnou kognitivní poruchou a zdravých subjektů byly srovnatelné nebo mírně lepší než v ostatních studiích, zatímco úspěšnost klasifikace pacientů se schizofrenií byla nižší než ve většině publikovaných studií a nebyla ani lepší než výsledky získané při klasifikaci dat analyzovaných standardními redukčními metodami, což je v práci diskutováno. Metoda penalizované lineární regrese s resubstitucí dále odhalila, že asociace oblastí mozku a kognitivních funkcí byly v datech zdravých dobrovolníků velmi nízké. Výsledkem práce je rovněž zjištění, že typ obrazových dat a výběr klasifikační metody neměly významný vliv na výsledky klasifikace, zatímco způsob provedení křížové validace výsledky značně ovlivnil. Závěr. Přestože dosažené výsledky jsou v případě rozpoznávání pacientů s Alzheimerovou chorobou slibné, dosažená úspěšnost bohužel stále není dostatečná pro aplikaci v klinické praxi. Výzkum by proto měl pokračovat i nadále, přičemž je nutné využívat co největších datových souborů a dbát na správné nastavení experimentů i křížové validace, aby se předešlo falešně pozitivním a zkresleným výsledkům.

Abstract:

Background. Diagnostics of mental disorders is still mainly based on observation and clinical interview which is subjective and sometimes not accurate. Computer-assisted classification methods based on brain imaging, cognitive or genetic data may have potential to make the diagnostics of these diseases faster and less subjective. However, such data are usually very large leading to unstable and often low classification performance. To overcome these problems, classification is preceded by data reduction but standard data reduction techniques have several shortcomings. Therefore, the aim of this thesis is to propose a fully automated multivariate method for efficient data reduction to enable accurate differentiating patients with mental disorders from healthy subjects. Methods. Two supervised multivariate whole-brain reduction techniques called penalized linear discriminant analysis with resampling and penalized linear regression with resampling were proposed and implemented in the ‘Penalised Reduction & Classification Toolbox’ for MATLAB. To evaluate their efficiency, the methods were applied on simulated and real data of patients with schizophrenia, Alzheimer’s disease and healthy controls in four experiments. Results. In all four experiments, the methods enabled detection of brain areas relevant to the studied disorders. Moreover, the simulated data reduced using these two methods were classified with the accuracy of almost 100 % and the classification performance on the data of patients with Alzheimer’s disease, mild cognitive impairment and healthy controls was comparable or slightly superior to related state-of-the-art findings. On the other hand, the performance of the classification of schizophrenia patients and healthy subjects was lower than in other state-of-the-art studies and the results were not significantly better than the performance obtained in classification of data reduced by standard reduction methods. The possible explanations are discussed. Furthermore, the penalized linear regression with resampling revealed that the effect size of the association of brain regions with cognitive functions in healthy individuals was very low. Besides, neither type of imaging features nor the classification method had an important influence on the classification results, whereas the setting of cross-validation proved to be the most influential factor in terms of the classification performance. Conclusions. Although the results in the Alzheimer’s disease recognition are promising, the accuracy of classification is still too low to be considered for clinical application. Thus, the research should continue with new studies using large samples and correct experiment and cross-validation settings to avoid false positive and biased results.
