Positron emission tomography for multiple myeloma

Abstract:

Má bakalářská práce pojednává o pozitronové emisní tomografii (PET), o jedné z nejmodernějších diagnostických metod, kterou může nukleární medicína poskytnout a o jejím přínosu v diagnostice hematoonkologického onemocnění mnohočetného myelomu. V teoretické části je rozebrán fyzikální princip PET, druhy používaných radiofarmak u PET, akvizice a rekonstrukce PET obrazů a následně je popsáno onemocnění mnohočetným myelomem - základní charakteristika, možnosti terapie a klinický obraz. V praktické části jsou zpracována data, která byla zjištěna v rámci výzkumu, o počtu vyšetřených na PET/CT z důvodu diagnostiky mnohočetného myelomu na Klinice nukleární medicíny ve FNO v období od ledna do prosince 2019. Z dat, která mi byla k dispozici bylo zjištěno, že podstoupilo vyšetření více mužů než žen, z celkového počtu 73 pacientů byla prokázána osteolytická ložiska u 49 vyšetřovaných, přičemž zvýšená metabolická aktivita v těchto ložiscích byla u 32 pacientů. Metabolická ložiska mimo kost v měkkých tkáních byla zjištěna pouze u 6 vyšetřovaných. PET/CT je tedy velikým přínosem jednak pro diagnostiku tohoto hematoonkologického onemocnění, ale též sleduje efekt léčby onemocnění, kdy právě pozitronová emisní tomografie pomáhá rozpoznat, zda lytické ložisko zjištěné na CT je či není metabolicky aktivní, a tudíž zda v rámci diagnostiky je přítomna viabilní nádorová tkáň, respektive lze hodnotit odezvu na provedenou terapii.

Abstract:

My thesis is centered around positron emission tomography (PET), which is one of the leading modern, diagnostic, methods that nuclear medicine can provide nowadays, and its contribution to the diagnosis of hematooncological disease of multiple myeloma. The theoretical part deals with the axiom of PET, the types of radiopharmaceuticals used in PET, the acquisition and reconstruction of PET images and finally, it describes the disease of multiple myeloma, including the basic characteristics, treatment options and clinical picture. In the practical part I am going to discuss and analyze data from my case study, which contains the number of patients tested for PET/CT based on the diagnosis of multiple myeloma by the Department of Nuclear Medicine at FNO in the timeframe from January to December 2019. According to the obtained data it was clear that in general more male subjects got tested then, female subjects. While taking a closer look at the numbers one can see that from the overall number of 73 tested individuals 49 were diagnosed with osteolytic deposits, while increased metabolic activity in these deposits could be found in the samples of 32 patients. Nevertheless, increased Metabolic deposits outside of the bone tissue, to be exact deposits that where located in the soft tissues, only related to 6 out of the 73 tested patients. Taking the above-mentioned information, it is fair to say that PET/CT is a very beneficial and effective method in regard to the diagnosis of this specific hematooncological disease. However, PET/ CT is also very beneficial when it comes to monitoring the treatment process, in which the emission tomography helps to determine whether the lytic lesion detected on CT is metabolically active. Thanks to this process one can determine if there is viable tumor tissue is present. On the other hand, one can also evaluate the effectiveness of the chosen therapy.
