Experience of patients with electroconvulsive therapy

Abstract:

ABSTRAKT Cíl: Hlavním cílem diplomové práce bylo získat jedinečný pohled na zkušenosti pacientů s elektrokonvulzivní terapií a zjistit jaké pocity prožívají v souvislosti s její aplikací. Metodika: Vzhledem ke stanovenému cíli byl výzkum koncipován jako kvalitativní studie. Výzkumný soubor tvořili čtyři muži, kterým byla v minulosti indikována elektrokonvulzivní terapie. Data získaná formou polostrukturovaných rozhovorů byla analyzována pomocí interpretativní fenomenologické analýzy. Výsledky: Na základě analýzy byla formulována následující témata zachycující zkušenosti participantů s elektrokonvulzivní terapií: souhlas s léčbou; vlastní aplikace ECT; nežádoucí účinky ECT; zlepšení zdravotního stavu po ECT; stigmatizace ECT a zlepšení jejího vnímání; ECT - ano či ne?; ztráta důvěry k pomáhajícím profesím. Ze získaných dat je zřejmé, že zkušenosti participantů jsou jedinečné, každý z nich reagoval na léčbu zcela jinak. Z rozhovorů vyplynula odlišná úroveň informovanosti o účincích ECT, rozdíly v průběhu vlastního ošetření i následné péče. Zjištění, týkající se žádoucích i nežádoucích účinků léčby odpovídají současným výzkumným poznatkům. Závěr: Čím vyšší je úroveň informovanosti pacientů a ošetřovatelské péče, tím je toto ošetření lépe snášeno. Pocitům obav a strachu však ani dostatečná informovanost nedokáže zabránit, ale zcela jistě dokáže zabránit pocitům zrady a frustrace v případě neočekávaného průběhu léčby a objevení se nežádoucích účinků, o kterých pacient nebyl předem zpraven. Proto je nutné, aby upřímná a citlivá komunikace byla nedílnou součástí péče o pacienty. Informování a vzdělávání veřejnosti o mechanismech a účincích ECT by bylo přínosné pro zlepšení vnímání a destigmatizaci této léčby.

Abstract:

ABSTRACT Aim: The main goal of this diploma thesis was to get a unique view on the experiences of patients with electroconvulsive therapy and to find out what feelings they experience in connection with its application. Methodology: Given the stated goal, the research was framed as a qualitative study. The research group consisted of four men who had been indicated for electroconvulsive therapy in the past. The data, obtained in the form of semi-structured interviews, were analyzed using interpretive phenomenological analysis. Results: Based on the analysis, the following topics were formulated capturing the participants' experiences with electroconvulsive therapy: consent to treatment; own ECT applications; side effects of ECT; improving health after ECT; stigmatizing ECT and improving it's perception; ECT - yes or no?; loss of trust in the helping professions. From the data obtained, it is clear that the participants' experiences are unique, each of them reacted to the treatment completely differently. The interviews showed a different level of awareness of the effects of ECT, differences during treatment and follow-up care. The data obtained, regarding the desirable and undesirable effects of treatment, correspond with current research findings. Conclusion: The higher the level of patient awareness and nursing care, the better the treatment is tolerated. However, even sufficient information cannot prevent feelings of anxiety and fear, but it can certainly prevent feelings of betrayal and frustration in the event of an unexpected course of treatment and side effects that the patient has not been notified of in advance. Therefore, honest and sensitive communication must be an integral part of the patient care. Greater awareness of the mechanisms and effects of ECT would be beneficial for improving the perception of this treatment.
