Prevence a detekce rizikových faktorů kognitivních a osobnostních změn u pacientů s Parkinsonovou nemocí léčených metodou hluboké mozkové stimulace.

Prevention and detection of risk factors for cognitive and personality changes in patients with Parkinson disease treated by deep brain stimulation

Abstract:

Hluboká mozková stimulace subthalamického jádra (DBS STN) je považována za klinicky přínosnou metodu léčby příznaků pokročilého stadia Parkinsonovy nemoci (PN). Cílem našeho výzkumu bylo ověřit vliv DBS STN na kognitivní funkce a identifikovat ukazatele (v oblasti kognitivních funkcí, kvality života a adaptačních strategií) na jejichž základě by bylo možné predikovat riziko rozvoje kognitivních změn po implantaci DBS STN. Do výzkumného souboru bylo zařazeno 46 pacientů (14 žen, 32 mužů) v době implantace DBS STN 59,61 let; SD=7,06). U všech zkoumaných osob bylo provedeno neuropsychologické zhodnocení kognitivních funkcí před a po implantaci (s časovým odstupem 23,46 měsíců; SD=7,07). Testová baterie byla tvořena neuropsychologickými testovými metodami (zkrácená verze WAIS-R, DRS, Seznam slov, ROCF, Stroopův test, verbální fluence, TOL), škálou MADRS, dotazníky PDQ-39 a SVF 78. Analýza srovnání rozdílů ve výkonech v neuropsychologických testech před a po implantaci DBS byla provedena Wilcoxonovým párovým testem pro závislé vzorky. K změření míry vztahu mezi proměnnými byl použit Spearmanův koeficient pořadové korelace. Potvrdili jsme hypotézy o negativním dopadu DBS STN na kognitivní funkce. Dle očekávání došlo ke snížení výkonů v oblasti verbální paměti, verbální fluence a v některých aspektech exekutivních funkcí (výkonového psychomotorického tempa, pozornosti, pracovní paměti, strategického uvažování). Z klinicko-psychologického úhlu pohledu však považujeme tyto změny za mírného charakteru, jelikož rozdíly průměrných hodnot výkonů naměřených v celém souboru před a po implantaci v testových metodách nepřekročily rozdíl -1,5 standardní odchylky pro daný skór testu. Jako citlivý prediktivní faktor kognitivních změn po DBS STN se ukázala kvalita vizuální paměti. Usuzujeme, že předoperační nízké výkony v oblasti vizuální paměti odráží progresi degenerativního procesu do dalších oblastí mozku mimo frontostriatální okruh, zejména temporoparietálních. V oblasti kvality života se jako citlivé ukazatele reagující na změnu kognitivních funkcí ukázaly PDQ-39 subškály Stigmatizace, Sociální podpora a Komunikace, v oblasti adaptačních strategií pak subškály Rezignace a Únikové tendence. Ve výzkumné práci jsme potvrdili negativní, avšak v klinicko-psychologickém kontextu mírný, dopad DBS STN na kognitivní funkce, a podařilo se nám nalézt prediktivní faktory ve vztahu ke kognitivním deficitům po DBS STN u idiopatické PN.

Abstract:

Deep brain stimulation of subthalamic nucleus (DBS STN) is considered a clinically established treatment method of symptoms of advanced stage of Parkinson's disease (PD). The aim of our research was to verify the effect of DBS STN on cognitive functions and identify the indicators (in the area of cognitive functions, quality of life and adaptation strategies) on the basis of which it would be possible to predict the risk of the development of cognitive changes after DBS STN implantation. The research group included 46 patients (14 women and 32 men) with an idiopathic PD diagnosis who underwent DBS STN implantation (average age at the time of the implantation was 59.61 years, SD = 7.06). A neuropsychological assessment of cognitive functions was performed on all subjects before and after the implantation (after 23.46 months; SD=7.07). Test battery consisted of neuropsychological tests (abbreviated version of WAIS-R, DRS, World list, Stroop test, verbal fluency tests, TOL), MADRS, questionnaires PDQ-39 and SVF-78. The comparison analysis of the performance differences in individual neuropsychological tests before and after DBS STN implantation was done using Wilcoxon Signed Ranks Test. Spearman´s Rank-Order Correlation was used to measure the relationship between the variables. We confirmed our hypothesis of a negative impact of DBS STN on cognitive functions. As we expected, there was a decrease in the performance in the area of verbal memory, verbal fluency and in some aspects of executive functions (psychomotor speed, attention, working memory and strategic thinking), however, the differences in the average performance values measured across the research group before and after the implantation in the tested methods did not exceed the difference of 1.5 of standard deviation for the score of the given test. Quality of visual memory proved as a sensitive predictive factor related to risk for cognitive changes after DBS STN implantation. We assume that pre-operation low performance in the area of visual memory reflects the degenerative process in Parkinson’s disease to other brain areas apart from the frontostriatal circuit, mainly to posterior temporoparietal areas. As regards quality of life, the sensitive indicators reacting to the changes of cognitive functions proved to be the PDQ-39 subscales Stigmatization, Social Support and Communication, in the area of adaptation strategies then Resignation and Escaping Tendencies. In our research work we confirmed a negative impact of DBS STN on cognitive functions, but only with mild severity in the clinical-psychological context, and we managed to find predictive factors in relation to cognitive deficits after DBS STN for PD.
