Knowledge of nurses working in the intensive care for external drains cerebrospinal fluid

Abstract:

Cíl: Cílem diplomové práce bylo zmapovat úroveň znalostí o péči o zevní komorové a lumbální drenáže u všeobecných sester pracujících v intenzivní péči a dále zmapovat způsob ošetřování těchto drenáží. Dílčí cíle zjišťovaly úroveň teoretických znalostí všeobecných sester o zevních komorových a lumbálních drenážích dle typu pracoviště, dle délky praxe a vzdělání. Konečnou etapou bylo vypracování doporučeného postupu péče o tyto drenáže. Metodika: Do výzkumného souboru bylo zařazeno 168 všeobecných sester pracující ve Fakultní nemocnici Ostrava, Městské nemocnici Ostrava a ve Fakultní nemocnici Olomouc. Rozhodujícím kritériem byl výskyt zevní komorové a lumbální drenáže na pracovišti. Výzkumné šetření probíhalo od září do prosince 2014 a informace byly získány pomocí dotazníku, který byl koncipován výzkumníkem. Následně byla získaná data statisticky zpracována. Výsledky: Výzkumným šetřením byly splněné stanovené cíle diplomové práce tím, že byla zjištěna úroveň znalostí všeobecných sester o externích drenážích mozkomíšního moku v souvislosti se zaměřením pracoviště, s délkou praxe a vzděláním. Statistickým ověřením stanovených hypotéz byl zjištěn významný rozdíl v souvislosti se vzděláním ve prospěch specializačního studia. V souvislosti s délkou praxe na ARO a JIP nebyl zjištěn významný rozdíl. V posudku týkajícího se zaměření pracoviště dosáhly neurochirurgické pracoviště statisticky významného rozdílu ve znalostech oproti ostatním pracovištím, kde se vyskytují tyto drenáže. Z celkového počtu respondentů tvořilo největší skupinu 49 (29 %) všeobecných sester, které dosáhly 66% úspěšnosti v odpovědích. Průměrná úspěšnost v odpovědích byla 65 %. Závěr: Pacient s drenáží, která umožňuje přístup do likvorových cest, jsou ohroženi řadou komplikací, které mohou v určitých situacích skončit až fatálně. Pacient se zevní komorovou nebo lumbální drenáží je doménou především neurochirurgických pracovišť, což je pravděpodobné vysvětlení získaných výsledků, nicméně existují situace výskytu drenáží i na jiných úsecích intenzivní péče a je důležité eliminovat komplikace erudovaným ošetřovatelským personálem a stanoveným postupem péče o tuto drenáž.

Abstract:

Objective: The primary aim of this thesis was to explore the intensive care nurses knowledge level concerning the external ventricular and lumbar drainage treatment. The secondary goal was to find out the common drainage treatment practice. Sub-objectives dealt with the nurses theoretical knowledge level concerning external ventricular and lumbar drainage in accordance with the type of work, the occupation span and the education level. The final phase embodied in the development of the drainage treatment systematic reference. Methods: The research sample included 168 nurses working at the Ostrava University Hospital, Ostrava City Hospital and the Olomouc University Hospital, that met the given criteria. The crucial point was the external ventricular and lumbar drainage incidence in the working place. The survey took place from September to December 2014. The data were collected by means of a questionnaire that was designed by a researcher. Subsequently they were statistically processed. Results: The thesis objectives were accomplished via the executed research. The nurses theoretical knowledge level concerning external cerebrospinal fluid drainage in accordance with the type of work, the occupation span and the education level was detected. The statistic significant differences were found between groups divided according to the education level in favor of specialized study. No statistic significant difference was find between groups divided according to the ARO and ICU experience span. In an opinion concerning the topics of department the neurosurgery departments had statistically significant better knowledge leves compared to other workplaces where these drainages occur. There were 49 (29 %) general nurses within the sample. They achieved 66% success in the answers. The average success in the responses was 65%. Conclusion: The patients with drainage allowing access to the CSF pathways are endangered by a number of complications that may end fatally in certain conditions. Patient with external ventricular or lumbar drainage is primarily the neurosurgical centers realm. This is a possible explanation of the results even if there is a drainage occurence on other areas of intensive care and it is important to eliminate complications knowledgeable nursing staff.
