Selected vitamin levels in pregnancy complicated by gestational diabetes mellitus

Abstract:

U onemocnění diabetes mellitus dochází vlivem hyperglykemie k aktivaci patofyziologických drah a zvýšení oxidačního stresu buněk, což má za následek rozvoj diabetických komplikací. Naproti tomu stojí systém potenciálně protektivních systémů, které se snaží negativní důsledky hyperglykemie alespoň částečně redukovat. Činností těchto protektivních systémů dochází ke zvýšení spotřeby aktivních forem vitaminů, mimo jiné také riboflavinu, které vystupují jako kofaktory účastnících se enzymů, případně vystupují jako antioxidanty. Gestační diabetes mellitus je jeden z typů diabetes, k jehož rozvoji dochází v období těhotenství a po porodu zaniká. Vzhledem k časové ohraničenosti tohoto onemocnění se jedná o vhodný model pro výzkum etiologie diabetes mellitus. Tato diplomová práce se zaměřuje na vypracování rešerše tykající se metabolismu vitaminů, především metabolismu riboflavinu, a jejich vztahu k diabetes mellitus, eventuelně gestačnímu diabetes mellitus. Dále zahrnuje vypracování rešerše shrnující metody pro stanovení riboflavinu, případně kofaktorů FAD a FMN, v biologických vzorcích. V experimentální části se věnuje vypracování a validaci metody stanovení riboflavinu, FAD a FMN ve vzorcích plazmy pomocí kapalinové chromatografie a jejímu využití pro stanovení plazmatických koncentrací riboflavinu, FAD a FMN u žen s gestačním diabetes mellitus a kontrolní skupiny žen s fyziologickým průběhem těhotenství s cílem sledované skupiny porovnat a případné změny koncentrací korelovat s dalšími biochemickými nebo antropometrickými parametry, které byly u jednotlivých probandů známy.

Abstract:

Diabetes mellitus is a disease characterized by hyperglycemia which activates pathophysiological pathways and increases oxidative stress in cells, resulting in the development of diabetic complications. In contrast, there are a potentially protective systems which can partially reduced the negative effects of hyperglycemia. The activity of these protective systems increases the consumption of active forms of vitamins, including riboflavin, which act as cofactors of participating enzymes, or act as antioxidants. Gestational diabetes mellitus is one of the types of diabetes that develops during pregnancy and disappears after childbirth. It is a suitable model for research about the etiology of diabetes mellitus due to the time dependence of this disease. This diploma thesis focuses on the elaboration of a search concerning the metabolism of vitamins, especially the metabolism of riboflavin, and their relation to diabetes mellitus or gestational diabetes mellitus. It also includes the elaborating of a search summarizing methods for the determination of riboflavin or coenzymes FAD and FMN in biological samples. The experimental part focuses on the development and validation of a method for the determination of riboflavin, FAD and FMN in plasma samples using liquid chromatography and its use for determining plasma concentrations of riboflavin, FAD and FMN of women with gestational diabetes mellitus and control groups of women with physiological course of pregnancy. Finally, possible changes in concentrations were corelated with other biochemical or anthropometric parameters that were known in individual probands.
