Controlled hypothermia in patients after cardiopulmonary resuscitation

Abstract:

Řízená hypotermie je v současnosti považována za perspektivní neuroprotektivní metodu poskytující ochranu před následky ischemického postižení zejména u pacientů po kardiopulmonální resuscitaci pro srdeční zástavu. Hypotermie vede ke snížení mortality a ke zlepšení kvality života u pacientů s hypoxickým poškozením mozku. V teoretické části popisuji terapeutickou hypotermii (TH) z pohledu historie, patofyziologie a následně popisuji její fáze, protektivní účinky, indikace a kontraindikace použití terapeutické hypotermie, jednotlivé ochlazovací metody a nakonec podpůrnou terapii ke zvládnutí vedlejších účinků. Průzkum v praktické části přináší přehled nemocničních pracovišť v Moravskoslezském kraji (MSK) provádějících terapeutickou hypotermii u pacientů po kardiopulmonální resuscitaci. V tomto přehledu jsou presentovány metody TH užívané na jednotlivých pracovištích, výskyt komplikací TH a jejich vztah k jednotlivým metodám TH. Dále v praktické části uvádím postoj ředitele územního střediska zdravotnické záchranné služby MSK k zavádění TH do přednemocniční péče (PNP) v tomto kraji. V závěru praktické části jsem porovnal jednotlivé metody TH a pokusil jsem se vybrat metodu vhodnou do PNP.

Abstract:

Controlled hypothermia is currently considered a promising method for providing protection to the neuroprotective effects against ischemic disability, particularly in patients undergoing cardiopulmonary resuscitation for cardiac arrest. Hypothermia leads to lower mortality and improve quality of life in patients with hypoxic brain damage. In the theoretical part describes therapeutic hypothermia (TH) from the perspective of history, pathophysiology, and then describe its stages, protective effects, indications and contraindications for therapeutic hypothermia, cooling the various methods and ultimately supportive therapy to manage side effects. A survey of the practical part provides an overview of hospital departments in the Moravian-Silesian Region (MSR) conducting therapeutic hypothermia in patients after cardiopulmonary resuscitation. In this report are presented the methods used TH in individual workplaces, TH incidence of complications and their relationship to various methods of TH. In another part of the practical part describes the position of Director of the Centre's regional medical rescue services MSR in the implementation of TH in pre-hospital care in this region. In conclusion the practical part, I compare the various methods of TH and I tried to choose method suitable to the pre-hospital care.
