Abstract:

Cílem bakalářské práce bylo zjistit význam mikrobiomu dýchacích cest u pacientů na umělé plicní ventilaci. Na základě stanovených cílů byly pro sběr dat použity vybrané elektronické databáze. Nalezené studie byly dále hodnoceny dle zvolených kritérií. V prvním cíli jsme se zaměřili na zmapování aktuálních dohledatelných poznatků významu komenzálního vztahu mikrobiomu na sliznici dýchacích cest. Výsledky uvádějí, že jsou dýchací cesty osídleny komenzálními bakteriemi, které mají ochranný a obranný charakter. Druhý cíl se soustřeďuje na sumarizaci poznatků o důsledcích respirační dysbiózy u dospělých hospitalizovaných pacientů na umělé plicní ventilaci. Výsledky nalezených studií hovoří jednoznačně nejen o významnosti plicního mikrobiomu v roli protektivního partnera na sliznici dýchacích cest, ale přináší důkazy o tom, že umělá plicní ventilace a zejména délka ventilačních hodin má vážné negativní důsledky na alfa-diverzitu plicního mikrobiomu a je jednou z příčin dysbiózy, která se stává rizikovým faktorem morbidity a mortality pacienta. Do klinické praxe je potřeba zavést diagnostické metody, které efektivně a rychle odhalí strukturu plicního mikrobiomu kriticky nemocného pacienta na umělé plicní ventilaci, které budou mít významný prognostický charakter.

Abstract:

The aim of the bachelor's thesis was to determine the importance of the microbiome of the airways in patients on artificial lung ventilation. Based on the set goals, selected electronic databases were used for data collection. The found studies were further evaluated according to the selected criteria. In the first goal, we focused on mapping the current traceable knowledge of the importance of the commensal relationship of the microbiome on the respiratory tract mucosa. The results indicate that the airways are populated by commensal bacteria that have a protective and defensive character. The second objective is focused on summarizing knowledge about the consequences of respiratory dysbiosis in adult hospitalized patients on artificial lung ventilation. The results of the studies found unequivocally speak not only about the importance of the lung microbiome in the role of a protective partner on the respiratory tract mucosa, but also provide evidence that artificial lung ventilation and especially the length of ventilation hours has serious negative consequences on the alpha-diversity of the lung microbiome and is one of the causes of dysbiosis, which becomes a risk factor for patient morbidity and mortality. There is a need to introduce diagnostic methods into clinical practice that will effectively and quickly reveal the structure of the lung microbiome of a critically ill patient on artificial lung ventilation, which will have a significant prognostic character.
