Nursing Process in Care of Patient after Implantation of the Aortic Valve by Using TAVI Method

Abstract:

Bakalářská práce je zaměřena na ošetřovatelský proces v péči o pacienta s významnou aortální stenózou podstupující implantaci aortální chlopně katetrizační metodou TAVI. Katetrizační implantace aortální chlopně představuje jednu z nových chirurgických metod využívané v kardiochirurgii u pacientů vysokého věku a pacientů s vysokým operačním rizikem. Cílem této práce je zpracovat názorný ošetřovatelský proces u vybraného pacienta s aortální stenózou podstupující implantaci chlopně výše zmíněnou katetrizační metodou. Ošetřovatelský proces je zpracován na základě ošetřovatelské diagnostiky dle NANDA International Taxonomie II. Práce je rozdělena na část teoretickou, která obsahuje dvě hlavní kapitoly a část praktickou. První kapitola popisuje problematiku aortální stenózy, jenž se zabývá etiologií, patofyziologií, klinickým obrazem, diagnostikou a možnou terapií onemocnění. Druhá kapitola je zaměřena na konkrétní intervenční výkon TAVI, popisující typy chlopní, průběh výkonu a možné pooperační komplikace. Také charakterizuje indikace, kontraindikace operační techniky a ošetřovatelskou péči o pacienta v pooperačním období. V praktické části je popsán ošetřovatelský proces u vybraného pacienta s aortální stenózou po náhradě aortální chlopně technikou TAVI. V rámci ošetřovatelského procesu byly u pacienta určeny ošetřovatelské diagnózy v předoperačním a pooperačním období. Na základě zjištěných diagnóz byly stanoveny konkrétní cíle a odpovídající ošetřovatelské intervence s popisem realizace a hodnocení péče u vybraného pacienta.

Abstract:

The bachelor thesis is focused on the nursing process in the care of the patient with significant aortic stenosis undergoing aortic valve implantation by using TAVI catheterization method. Aortic Valve Catheter Implantation is one of the new surgical methods used in cardiac surgery in elderly patients and patients at high operational risk. The aim of this work is to elaborate an illustrative nursing process in a selected patient with aortic stenosis undergoing valve implantation by the catheterization method. Nursing process is based on nursing diagnostics according to NANDA International Taxonomy II. The thesis is divided into a theoretical part, which contains two main chapters and a practical one. The first chapter describes the issue of aortic stenosis, which deals with etiology, pathophysiology, clinical picture, diagnosis and possible therapy of the disease. The second chapter is focused on specific intervention TAVI, describing the types of valves, the course of performance and possible postoperative complications. It also characterizes indications, contraindications of surgical techniques and nursing care for patients in the postoperative period. In the practical part is described nursing process in selected patient with aortic stenosis after aortic valve replacement by TAVI technique. In the nursing process, nursing diagnoses in the preoperative and postoperative period were determined for the patient. Based on the diagnoses identified, specific goals and corresponding nursing interventions with the description of the implementation and evaluation of care for the selected patient were established.
