Abstract:

Tématem bakalářské práce je sociální práce s rodinou v paliativní péči. Práce obsahuje teoretickou a aplikační část. Teoretická část se skládá ze tří kapitol, ve kterých se zaměřuji na paliativní péči, sociální práci a na rodinu. V první kapitole se věnuji popisu paliativní péče, jejích zásad a principů a jejího rozdělení. Druhá kapitola se zabývá sociální prací v paliativní péči, rolí a náplní práce sociálního pracovníka, zprostředkováním finanční podpory, navazujících služeb, dále psychosociální podporou a spoluprací multidisciplinárního týmu. Třetí kapitola se týká rodiny v paliativní péči, její potřeby, dále komunikace se sociálním pracovníkem společně s prvním kontaktem, doprovázením a prací s pozůstalými. Cílem mé bakalářské práce je popsat sociální práci s rodinou v průběhu paliativní péče. Aplikační část se skládá z analýzy kazuistiky pacienta v mobilním hospici, kde jsem zpracovala data ze spisové dokumentace s ohledem na etickou klauzuli. Cílem mé aplikační části je popsat sociální práci v domácím hospici sv. Michaela v Poličce při péči o klientku, která prochází nevyléčitelným onemocněním. Zaměřuji se na roli a činnosti sociální pracovníka v domácí hospicové péči u konkrétní klientky.

Abstract:

The theme of the bachelor's thesis is social work with a family in palliative care. The work contains a theoretical and application parts. The theoretical part consists of three chapters in which I focus on palliative care, social work and family. In the first chapter, I dedicate myself to describing palliative care, its principles and principles, and its division. The second chapter deals with social work in palliative care, the role and duties of a social worker, the mediation of financial support and downstream services as well as psychosocial support and the cooperation of a multidisciplinary team. The third chapter concerns a family in palliative care, its need, as well as communication with a social worker together with first contact, accompanying and working with survivors. My bachelor thesis aims to describe social work with my family during palliative care. The application part consists of an analysis of a patient's case report in a mobile hospice, where I processed data from the file documentation with respect to the ethical clause. The aim of my application part is to describe social work in the home hospice of St. Michael in Polička while caring for a client who is going through an incurable disease. I focus on the role and acrivities of a social worker in home hospice care for a particular client.
