Intake of vitamin D in patients after laparoscopic sleeve gastrectomy

Abstract:

Úvod: Zkušenosti posledních několika let dokazují, že výskyt morbidní obezity a jejich přidružených onemocnění stále roste. Jedním z nejefektivnějších způsobů léčby je bariatrická chirurgie. Proběhlo několik studií zabývajících se změnami kostní denzity a deficiencemi nutrientů, ale většina pouze s odkazem na malabsorpční výkony. Laparoskopická tubulizace žaludku je restriktivní zákrok, při němž dochází k zásahu do anatomie trávicího traktu a tak se dají i zde očekávat metabolické změny. Cíl: Cílem této práce bylo porovnat změny sérových hladin vitamínu D a dalších parametrů kostního metabolismu v krvi u obézních pacientů před podstoupením LSG a tři a šest měsíců po operaci. Metody: Studie proběhla u 44 morbidně obézních pacientů. V rámci vyšetření před zákrokem a tři a šest měsíců po něm, byl posuzován vliv tubulizace žaludku na tělesnou hmotnost, obvod pasu a boků a celkové množství a rozložení tělesného tuku (duální rentgenová absorpciometrie - DXA). Byla analyzována sérová hladina vitamínu D a některé další biochemické parametry a stanoveny případné deficity. U šesti pacientů byly podrobně rozebrány stravovací návyky a byly propočteny jejich jídelní záznamy ve výživovém softwaru Nutridan. Výsledky a závěr: Z výsledků je zřejmé, že pacienti trpěli závažným nedostatkem vitamínu D již před operačním výkonem. Půl roku po LSG činila ztráta tělesné nadhmotnosti (EWL) respondentů téměř 58,27 %, snížilo se jim BMI, obvod pasu i boků. Operace byla vyhodnocena jako úspěšná. Sérová hladina vitamínu D 3 měsíce po operaci stoupla na průměrnou hodnotu 46,607 nmol/l, díky významné ztrátě procenta tělesného tuku, ale stále je velmi nedostatečná. U šesti jedinců bylo díky jídelním záznamům prokázáno, že mají nedostatek vitamínu D a vápníku v přijímané stravě. Pacientům po LSG, by měla být doporučena suplementační léčba vitamínem D po dobu jednoho roku až dvou let od operace, aby došlo k navýšení sérových hladin vitamínu D v krvi.

Abstract:

Introduction: Experience from the past few years has shown that the incidence of morbid obesity and its associated diseases is growing. One of the most effective treatments is bariatric surgery. There have already been several studies dealing with changes in bone mineral density, as well as nutrient deficiency, but most of them only with reference to the malabsorptive types of procedures. Laparoscopic sleeve gastrectomy is a restrictive procedure which involves interference with the anatomy of the digestive tract and can be connected with metabolic changes. Design: The aim of this study was to compare changes in serum levels of vitamin D and other parameters of bone metabolism in the blood of obese patients before undergoing LSG and three and six months after the surgery. Methods: The study was performed on 44 morbidly obese patients. The examination before the surgery and three and six months after it evaluated the effect of sleeve gastrectomy on body weight, waist and hips, and the total amount and distribution of body fat (Dual X ? ray Absorptiometry ? DXA). The serum levels of vitamin D were analyzed together with other biochemical parameters, and possible deficits were identified. Six patients were analyzed in detail as far as their eating habits were concerned, and their dining entries were calculated in NutriDan nutritional software. Results and conclusion: The results show that patients had suffered severe deficiency of vitamin D prior to the surgery. Six months after LSG, the excess weight loss (EWL) was about 58,27 % and BMI, waistline and hips were reduced. The operation was evaluated as successful. Three months after the surgery, the serum level of vitamin D increased to the average of 46,607 nmol/l, due to the significant loss of body fat percentage, but it is still insufficient. Moreover, the dining entries of six people have proved a deficiency of vitamin D and calcium in their diet. Patients after LSG should be recommended a vitamin D supplementation therapy for one or two years after the surgery in order to increase serum levels of vitamin D in the blood.
