Physiotherapy in a client with carpal tunnel syndrome

Abstract:

Cílem práce je přispět k řešení problematiky fyzioterapie u pacienta se syndromem karpálního tunelu volbou vhodných fyzioterapeutických postupů, konceptů a metod. V bakalářské práci jsem zpracovala kazuistiku pacientky po operaci syndromu karpálního tunelu. Provedla jsem vstupní kineziologický rozbor, za použití metod rozhovoru, aspekce, palpace, měření a testování, dle kterého jsem následně stanovila krátkodobý fyzioterapeutický plán. Pomocí rozhovoru jsem se s pacientkou domluvila na spolupráci a odebrala jsem anamnézu. Aspekcí jsem vyšetřila držení, konfiguraci a trofiku, kterou jsem následně palpačně ověřila. Palpací jsem vyšetřila hybnost, tonus a svalovou sílu. Změřila jsem délky a obvody horních končetin a rozsahy pohybů. Testovala jsem speciální testy na horní končetinu, statické a dynamické úchopy ruky dle Kapanjiho, povrchovou a hlubokou senzitivitu, spastické a paretické jevy, reflexy a provokační manévry. Proběhlo celkem 12 fyzioterapeutických intervencí v období od 8.9.2020 do 14.12.2020 z čehož tři pacientka absolvovala před operací a zbylých devět po operaci. V jednotlivých terapiích jsem využila různé fyzioterapeutické postupy, koncepty a metody, jako jsou: ACT, Brügger koncept, PFS, PNF, Spiraldynamik, měkké a myofasciální techniky, míčková facilitace, výcvik úchopů a jemné motoriky a aktivní, pasivní pohyby a pohyby proti odporu. Během poslední terapie jsem provedla výstupní kineziologický rozbor, na jehož základě jsem stanovila dlouhodobý fyzioterapeutický plán, se kterým jsem pacientku detailně seznámila. Po absolvování komplexní fyzioterapie po operaci syndromu karpálního tunelu došlo ke snížení bolesti a otoku ruky až o 1-2 cm. Dále se zvětšil rozsah pohybu v radiokarpálním kloubu (RC) do radiální dukce (RD) a do palmární flexe (PF) o 5°a do ulnární dukce (UD) a dorsální flexe (DF) o 10°a v drobných kloubech prstů do FL. Došlo k zvýšení svalové síly v RC kloubu do RD, UD, PF a DF až o 1 stupeň, v MP, PIP a DIP kloubech do FL a EXT o 0,5 - 1,5 stupně. Dále došlo k obnově úchopové funkce ruky. Běžné denní činnosti pacientce nečiní žádné obtíže a subjektivně cítí úlevu od bolestí a omezení. Fyzioterapie byla u pacientky úspěšná, neboť se nám podařilo dosáhnout vytyčených cílů a drobné nedostatky by měly být odstraněny pomocí autoterapií v rámci dlouhodobého fyzioterapeutického plánu.

Abstract:

The aim of this work is to contribute to the solution of physiotherapy in patients with carpal tunnel syndrome by choosing appropriate physiotherapeutic procedures, concepts and methods. In the bachelor thesis I processed a case report of patient after surgery of carpal tunnel syndrome. I made an initial kinesiological analysis, using the metods of interview, aspect, palpation, measurement and testing, according to which I prepared a short-term physiotherapy plan. During the interview i took a medical history and the patient agreed to cooperate with me. I visually examined the posture, configuration, and trophic, which then I palpated. I palpated the momentum, tonus and muscle strength. I measured the lengths and girths of the upper limbs and the range of motion. I tested special tests for the upper limb, static and dynamic hand grips according to Kapanji, surface and deep sensitivity, spastic and paretic phenomena, reflexes and provocative maneuvers. Twelve physiotherapeutic therapies were performed from 8.9.2020 to 14.12.2020, of which three were before surgery and nine were after surgery. I used various physiotherapeutic procedures, concepts and methods in individual therapies, such as ACT, Brügger concept, PFS, PNF, Spiraldynamik, soft and myofascial techniques, ball facilitation, grip training and fine motor skills and movements active, passive and against resistance. During the last therapy, I performed a final kinesiological analysis, on the basis of which I made a long-term physiotherapy plan, which I introduced to the patient. After complex physiotherapy after carpal tunnel syndrome surgery, the pain and swelling of the hand were reduced by up to 1-2 cm. The range of motion in the radiocarpal joint (RC) to adduction and palmar flexion was increased by 5 ° and to abduction and dorsal flexion by 10 ° and in small finger joints to flexion (FL). Muscle strenght was increased to all movements by up to 1 degree, in small finger joints to FL by 0,5 - 1,5 degrees. Furthermore, the gripping function of the hand was restored. Activities of daily living do not cause the patient any problems and subjectively feel pain relief. Physiotherapy was successful, because we managed to achieve the set goals and minor deficiencies should be eliminated through autotherapy as part of a long-term physiotherapy plan.
