Post-traumatic growth after childbirth

Abstract:

Porod je nejen v České republice bohužel stále poměrně často spojen s traumatickými zážitky, které u přibližně tří procent žen vyústí v posttraumatickou stresovou poruchu. Práce se věnuje posttraumatickému růstu u žen po porodu a částečně také kontextu, proč vůbec bývá porod spojen s traumatickými zážitky či posttraumatickou stresovou poruchou. V empirické části je analýza výpovědí šesti žen, které zažily posttraumatickou stresovou poruchu a následně také posttraumatický růst. Polostrukturované rozhovory jsou analyzovány pomocí interpretativní fenomenologické analýzy. Mezi příčinami vzniku posttraumatické stresové poruchy uvádějí ženy nejčastěji tzv. "porodnické násilí" ? medicínské zákroky, které jsou prováděny rutinně, necitlivě, často bez souhlasu či vědomí rodičky, a způsobem, který ji dostává do nedůstojného postavení; ukončení porodu akutním císařským řezem; několikahodinovou separaci zdravé matky a zdravého dítěte; a komunikaci lékařů či zdravotnického personálu s rodící ženou. Posttraumatický růst se nedostavil automaticky, ale ženy se k němu musely dopracovat např. pomocí psychoterapie a další práce na sobě. Růst se projevoval různými způsoby, např. nalezením vnitřní síly; zvýšením asertivity a schopnosti se za sebe postavit; změnou hodnot; změnami ve vztazích s rodinou, přáteli i s lidmi obecně; ve směřování kariéry či angažovanosti v oblasti dobrovolnictví.

Abstract:

Unfortunately, not only in the Czech Republic childbirth is still relatively often associated with traumatic experiences, which in three percent of women result in posttraumatic stress disorder. The thesis focuses on post-traumatic growth in women after childbirth and also on the context of the association of childbirth with traumatic experiences or post-traumatic stress disorder. The empirical part analyses the testimonials of six women who experienced post-traumatic stress disorder and subsequently post-traumatic growth. Semi-structured interviews are analyzed using interpretive phenomenological analysis. Among the causes of posttraumatic stress disorder, women most often mention so-called "obstetric violence" - medical interventions that are performed routinely, insensitively, without the consent or knowledge of the mother and in a way that puts her in an undignified position; the termination of labor by acute caesarean section; several hours of separation of a healthy mother and a healthy child; communication between doctors or medical staff and the woman in labor. Post-traumatic growth did not appear automatically, but women had to work towards it, for example, through psychotherapy and other methods of trauma mitigation. Post-traumatic growth manifested itself in various ways, such as finding personal strength, increased assertiveness and the ability to stand up for oneself, change of values, changes in relationships with family, friends, and people in general, new career paths, or involvement in volunteer work.
