Sequencing analysis of Treponema pallidum subsp. pallidum isolated from clinical material

Abstract:

Úvod. Syfilis je onemocnění, které způsobuje bakterie Treponema pallidum subsp. pallidum (TPA), a které může vést k celkové systémové infekci lidského těla. Prevalence syfilis v posledních desítkách let stoupá, a to i přes existenci efektivní léčby, a celosvětová incidence dosahuje až 10 milionů případů ročně. TPA není možno studovat pomocí klasických mikrobiologických přístupů, protože tato bakterie není kultivovatelná in vitro. Avšak revoluční techniky označované jako „culture-independent enrichment techniques“ otevírají novou kapitolu ve výzkumu treponem, neboť umožnují analyzovat genetickou informaci TPA bakterií izolovaných přímo z pacientů, a to bez nutnosti multiplikace těchto bakterií v experimentálních zvířatech. Metody. V rámci studie bylo na klinické vzorky aplikováno několik „culture-independent enrichment” technik, zahrnující “Anti-Treponemal Antibody Enrichment Sequencing” (ATAES), mechanickou mikromanipulaci, selektivní lyzi buněk, hybridizaci na oligonukleotidových DNA čipech a metody využívající selektivní rozpoznávání metylačních profilů v DNA pomocí DpnI endonukleázy. Pro charakterizaci molekulárních typů u kmenů způsobujících syfilis bylo využito 455 klinických vzorků z České republiky, Argentiny, Francie a Švýcarska. Klinické vzorky byly charakterizovány pomocí tří nezávislých molekulárně typizačních systémů včetně „Sequencing-based molecular typing” (SBMT), „Enhanced CDC-typing” (ECDCT) a nově navrženého systému „Multi-Locus Sequence Alignment Typing” (MLSAT). Výsledky. Pomocí ATAES se podařilo získat 2 nekompletní genomy TPA, 18 nekompletních genomů TPA bylo získáno pomocí hybridizace na DNA čipech a 5 nekompletních genomů TPA bylo získáno pomocí metody využívající selektivní rozpoznávání metylačních profilů v DNA s využitím DpnI endonukleázy. Získaná data sloužila pro identifikaci dvou geneticky rozdílných skupin treponem, které způsobují syfilis, a pro vývoj nového molekulárního typování bakterie TPA (MLSAT). Pomocí molekulární typizace byly charakterizovány současné TPA kmeny kolující v lidské populaci, byla popsána jejich celosvětová distribuce, jejich asociace s klinickou manifestací pacientů a byla identifikována prevalence kmenů, které jsou rezistentní vůči makrolidovým antibiotikům. Závěr. Poprvé byly použity „culture-independent enrichment” techniky spolu se sekvenováním nové generace pro analýzu klinických vzorků syfilis a byl zvýšen celkový počet genomů bakterie TPA (25 nekompletních genomů, které byly izolovány přímo z pacienta). Bylo navrženo nové typizační schéma, které jasně identifikuje dichotomní charakter bakterie TPA způsobující syfilis a mapuje genetickou variability uvnitř obou geneticky odlišných skupin – SS14-like a Nichols-like izolátů.

Abstract:

Introduction. Syphilis is a bacterial infection causing system damage to the body through the dissemination of the agent Treponema pallidum subsp. pallidum (TPA). Despite the availability of effective treatment, last few decades have witnessed a dramatic global re-emergence of syphilis, with annual worldwide incidence reaching more than 10 million. TPA cannot be studied using traditional microbiological approaches since this bacterium cannot be cultivated under in vitro conditions. However, the culture-independent enrichment of treponemal DNA in syphilis clinical samples opened a new chapter in Treponema research by enabling to obtain genetic data of TPA bacteria isolated directly from the patients, without the need of multiplication of TPA strains in experimental animals. Methods. Several approaches of culture-independent enrichment of TPA were applied to syphilis clinical samples including Anti-Treponemal Antibody Enrichment Sequencing (ATAES), mechanical micromanipulation, enrichment based on selective cell type lysis, DNA-capture microarray and DNA binding enrichment using restriction endonuclease DpnI. To describe current molecular types of syphilis causing strains from 455 clinical samples collected in the Czech Republic, Argentina, France and Switzerland, we used Sequencing-based molecular typing (SBMT), Enhanced CDC-typing (ECDCT) and newly developed Multi-Locus Sequence Alignment Typing (MLSAT). Results. We obtained two, eighteen and five draft genomes of clinical samples using ATAES, DNA-capture microarray and DNA binding enrichment using restriction endonuclease DpnI, respectively. The genetic data obtained by these techniques helped us to identify two genetically distinct groups of syphilis treponemes and developed a new molecular typing scheme of TPA (MLSAT). Moreover, we described the molecular types of treponemes, which represent the current syphilis infections, their worldwide distribution, their association with the clinical features of patients and revealed the prevalence of macrolide resistance in syphilis-causing strains. Conclusion and significance. We have introduced for the first time the culture-independent selective bacterial DNA enrichment techniques coupled with Next Generation Sequencing (NGS) to the analysis of syphilis clinical samples and increased a number of TPA genomes (25 draft genomes isolated directly from the patients). Moreover, we developed a new molecular typing scheme of TPA, which clearly identified the dichotomy found among TPA clinical samples and mapped genetic variability within both genetically distinct groups - SS14-like and Nichols-like TPA isolates.
