Nursing care map for a patient/client with liver cirrhosis

Abstract:

Úvod: Bakalářská práce je věnována komplexní ošetřovatelské péči o pacienta s jaterní cirhózou. Jaterní cirhóza je konečným stadiem chronického onemocnění jater a bývá příčinou předčasného úmrtí, zkracuje život cca o 10 let. Cíl práce: Cílem bakalářské práce je sestavení mapy ošetřovatelské péče o pacienta s jaterní cirhózou, kdy budou identifikovány a diagnostikovány ošetřovatelské problémy - diagnózy pacientů s diagnostikovanou jaterní cirhózou za použití NANDA taxonomie II. Metodika: Soubor tvořili 3 pacienti s diagnostikovanou jaterní cirhózou na interním oddělení. Sběr dat probíhal pozorováním, rozhovorem s pacientem, který dal anonymní souhlas ke zpracování údajů (příloha A), a byl obohacený o informace ze zdravotnické dokumentace. Při získávání dat byl použit ošetřovatelský koncepční model Marjory Gordon. Výsledky: Na základě vypracovaných kazuistik jednotlivých pacientů byly diagnostikovány jejich ošetřovatelské problémy, kterými byly akutní bolest, průjem, únava, zvýšený objem tekutin, neefektivní vzorec dýchání, riziko infekce a riziko krvácení. Na základě těchto problémů byla zpracována mapa ošetřovatelské péče za použití NANDA taxonomie II. Závěr: Zpracovaná mapa ošetřovatelské péče může být použita při příjmu pacienta s diagnostikovanou jaterní cirhózou a v průběhu celé jeho hospitalizace. Účelem je zkvalitnění komplexní ošetřovatelské péče.

Abstract:

Introduction: Thesis is dedicated to the complex nursing care of a patient with liver cirrhosis. Liver cirrhosis is the final stage of the chronic liver disease and is the cause of premature death, it shortens life by 10 years. Goal of the thesis: The goal of the thesis is to create a map of nursing care by a patient with liver cirrhosis, where there will be identified and diagnosed nursing problems - diagnosis of patients with diagnosed liver cirrhosis by using NANDA taxonomy II. Methodology: The set was formed by 3 patients with diagnosed liver cirrhosis in the internal department. The collection of the data was done by observation, dialogue with the patient with his anonymous approval to process the data (attachment A), and was enriched by the information from the medical documentation. By the collection of the data there was used the nursing concept model Marjory Gordon. Results: Based on the elaborated case studies of the individual patients there were diagnosed their nursing problems which were acute pain, diarrhea, fatigue, increased liquid volume, ineffective breathing pattern, risk of infection and risk of bleeding. Based on these problems there was processed the map of nursing care by using the NANDA taxonomy II. Conclusion: The processed map of nursing care can be used by the patient reception with diagnosed liver cirrhosis and by the course of the whole hospitalization. The purpose is the improvement of the complex nursing care.
