Influence of percieved social support on cognitive decline rate in community-dwelling seniors

Abstract:

ÚVOD: Stárnutí populace s sebou přináší zvýšení prevalence kognitivních poruch a demencí s významnými sociálními a ekonomickými důsledky. Při absenci účinné kauzální a nemoc modifikující léčby demencí je vhodné upřít pozornost k nalezení postupů zaměřených na prevenci. Základní podmínkou pro preventivní úsilí je znalost rizikových faktorů demencí. Míra percipované sociální opory je významným, potenciálně ovlivnitelným rizikovým faktorem kognitivního deficitu. METODY: 3,5 roku byla sledována kohorta 104 mladších seniorů žijících ve venkovské komunitě. Na úvod a na závěr sledování byla zjišťována úroveň subjektivně vnímané sociální opory a stav kognitivních funkcí. Při statistickém vyhodnocení byli probandi rozděleni na tercily dle úrovně sociální opory a byly zkoumány rozdíly v kognitivní výkonnosti mezi tercily na počátku sledování a v dynamice úbytku kognitivních funkcí mezi tercily za dobu sledování. VÝSLEDKY: Již na počátku studie byla zjištěna statisticky signifikantně horší úroveň kognitivních funkcí u probandů s nízkou úrovní sociální opory (1. tercil) oproti probandům s vysokou úrovní sociální opory (3. tercil), nepodařilo se však prokázat rozdíl v dynamice úbytku kognitivních funkcí v mezi tercily v longitudinálním sledování. Příčinou tohoto výsledku byl zřejmě významný přesun subjektů výzkumu mezi tercily sociální opory za dobu trvání studie. Toto zjištění je v souladu s prokázanou korelací výkonu v kognitivních testech se směrem přesunu probandů (přesun k nižší míře sociální opory znamenal zhoršení výkonu v některých kognitivních testech) ZÁVĚR: Výzkumná práce prokázala význam nízké sociální opory jako rizikového faktoru vývoje kognitivního deficitu u seniorů žijících ve venkovské komunitě.

Abstract:

BACKROUND: Population ageing rises prevalence of cognitive impairment and dementia among elderly with important social and economical consequences. Because of lack of effective causal or disease-modifying treatment of dementias, searching for preventive methods seems to be eligible. This effort requires good knowledge of dementias´ risk factors. The level of self-perceived social support is an important and potentially modifiable risk factor of cognitive decline. METHODS: Cohort of 104 rural community-dwelling younger seniors was observed for 3.5 years. Subjective perception of social support and cognitive functions were assessed in the beginning and in the end of the period. Research subjects were divided into three tertiles according to their level of self-perceived social support. Differences in cognitive performance among the tertiles at startpoint, and in dynamics of cognitive decline between tertiles in follow-up were identified during statistical evaluation.+ RESULTS: There was found statistically significant decrease of cognitive functions in subjects with low self-perceived social support (1st tertile) compared to subjects with high self-perceived social support (3rd tertile) in the beginning of the research study, but differences in dynamics of cognitive decline between tertiles in follow-up were not substantial. This result might be caused by the switch of some subjects to different tertiles during the follow-up. This is supported by the finding of correlation of cognitive performance level and the direction of subject´s shift within the social support tertiles (shift to lower level of social support was accompanied by worsening of cognitive performance in certain cognitive tests) CONCLUSION: Research results indicate the significance of social support as a risk factor for developing of cognitive decline in rural community-dwelling seniors.
