The palliative care in hospic and the barriers to accompany the dying.

Abstract:

Dnešní doba se snaží utrpení a smrt co nejvíce vytěsnit a těžko si přiznáváme, že vůči některým chorobám jsme slabí, že někdy medicína svůj boj prostě prohraje. Nemoc a smrt chápeme jako prohru, jako velkou tragedii a jen těžko dokážeme připustit, že v lidském životě mají své místo nebo dokonce i smysl a právě k tomu, aby pacient tento smysl mohl nalézt, slouží komplexní přístup paliativní péče a doprovázení, na které však musí být také dostatek času. Práce přináší souhrn poznatků a praktických zkušeností, ve výzkumu se zaměřuje na tři hlavní překážky v paliativní péči a doprová-zení. To jsou: nedostatečné informace o nemoci, absence ( i obecné) paliativní péče v nemocnicích a zařízeních následné péče a pozdní příchod pacienta do hospice (ante finem, in finem). Cílem této práce je poukázat na výše zmíněné nedostatky a také poukázat na pnutí mezi možnostmi léčby aplikované v hospici a realitou jiných zařízení. Sestává ze čtyř kapitol. První je věnována všeobecným informacím, historii, vývoji a současnosti paliativní péče u nás, druhá je medicínská, přináší základní informace o paliativní péči v onkologii a o symptomech, které léčí. Je velmi obsáhlá, ačkoliv informace v ní jsou značně zredukované. Další dvě kapitoly náleží do praktické části. Třetí kapitola je věnována hospici Anežky České v Červeném Kostelci, péči o nemocné a doprovázení a čtvrtá kapitola se zaobírá výzkumem.

Abstract:

There is lot of people today they try to exclude the suffering and death out of life. They are not able to accept their weakness to incurable disease, to accept the defeat of medicine. They perceive disease and death like the big tragedy and they are not able to recive it like the part of human‘s life. The palliative care tries to help the terminally ill to find the sense of the life, suffering and death accepting all the needs of human being. The care and accompanying the dying needs to take some time. This thesis submits the theoretical knowledge and practical experience in palliative care and accompanying of dying and the problems and barriers to apply it fully for all the patients. There are three main problems: Insufficient information of the patient about the illness, the absence of palliative care in hospitals and other institutions and the late arrival of patients to hospic. The main purpose of my research is to submit the problems and barriers to provide the palliative care for all the patients coming to hospic bringing forward the options of palliative care applied in hospic. The thesis includes four chapters. The first carries the basic and historical dates about the palliative and hospic care, the second one brings the medical aspects of palliative oncological care and treatment of symptoms. The third chapter talks about Hospic of Agnes the Czech in Červený Kostelec and the palliative care experience there. The fourth chapter puts in the research.
