Evaluating efficacy of training the inspiratory muscles in patients with chronic pulmonary disease

Abstract:

Chronická respirační onemocnění, mezi která řadíme chronickou obstrukční plicní nemoc, bronchiální astma i idiopatickou plicní fibrózu, se od svého vzniku stávají celoživotním zdravotním problémem, který svými projevy značně ovlivňuje fyzický i psychický stav nemocného. Pro tato onemocnění je charakteristický výskyt respiračních symptomů, a to zejména dušnosti a kašle provázeného bolestmi na hrudníku, jimiž je limitováno provádění běžných denních činností v životě pacienta. Snahou respirační fyzioterapie, jejíž součástí je i trénink dýchacích svalů s využitím dechových trenažérů, je omezit výskyt symptomů, zpomalit progresi onemocnění a zlepšit kvalitu života nemocného. Cílem této studie je zhodnotit efekt tréninku nádechových svalů s dechovou pomůckou Threshold IMT u nemocných s chronickou obstrukční plicní nemocí (CHOPN), bronchiálním astmatem (AB) a idiopatickou plicní fibrózou (IPF). Probandi byli rozděleni do tří skupin podle svého onemocnění a následně podstoupili 8týdenní inspirační trénink v domácím prostředí. Sledovány byly jejich spirometrické parametry, maximální okluzní ústní tlaky, rozvíjení hrudníku a vnímání dušnosti dle modifikované škály dušnosti (mMRC). Součástí práce je i meziskupinové srovnání výsledných hodnot a korelační analýza u vybraných parametrů. U skupiny pacientů s CHOPN došlo ke statisticky významnému zlepšení maximálního výdechového ústního tlaku (p=0,016 dle párového t-testu), u skupiny pacientů s AB a IPF došlo k signifikantnímu zlepšení síly nádechových svalů (p=0,022 a p=0,014) a zároveň se u skupiny IPF prodloužila doba nádechu a výdechu během celkového dechového cyklu (p=0,006). U všech skupin bylo prokázáno signifikantní zlepšení rozvíjení hrudníku na dvou úrovních hrudního koše - mesosternale a xiphosternale. Míra dušnosti se statisticky významně snížila u skupiny pacientů s CHOPN a IPF (p=0,001 a p=0,012) a u skupiny AB se statistický rozdíl blížil hladině významnosti (p=0,052). Z výsledků je tedy zřejmé, že by měl být trénink nádechových svalů důležitou součástí komplexní terapie u pacientů s chronickým plicním onemocněním.

Abstract:

Chronic respiratory diseases, which include chronic obstructive pulmonary disease, bronchial asthma, and idiopathic pulmonary fibrosis, are the cause of lifelong health problems ever since they start. The symptoms of these diseases affect both the physical and mental health of patients. Typical respiratory symptoms are especially dyspnea and cough associated with chest pain. This is the reason the patient is limited in doing an activity of daily living. The aim of respiratory physiotherapy, which also includes the training of respiratory muscles with the use of breathing devices, is to reduce the occurrence of symptoms, slow down the progression of the disease and improve the quality of life of the patient. The aim of this study is to evaluate the effect of respiratory muscle training with the Threshold IMT in patients with chronic obstructive pulmonary disease (COPD), bronchial asthma (AB), and idiopathic pulmonary fibrosis (IPF). The probands were divided into three groups according to their illness and then they did eight weeks of inspiratory training at home. Their spirometric parameters, maximal mouth occlusion pressures, chest development, and perception of dyspnea according to the Modified Medical Research Council dyspnea scale (mMRC) were monitored. A part of the master thesis is also an intergroup comparison of the resulting values and correlation analysis for selected parameters. In the group of patients with COPD there was a statistically significant improvement in maximal expiratory mouth pressure (p=0.016 according to paired t-test), in the group of patients with AB and IPF there was a significant improvement in inspiratory muscle strength (p=0.022 and p=0.014) and in the IPF group, the inspiratory and expiratory time was prolonged during the total respiratory cycle (p=0.006). All groups showed a significant improvement in chest development at two levels of the thorax - mesosternal and xiphosternal. The rate of dyspnea decreased statistically significantly in the group of patients with COPD and IPF (p=0.001 and p=0.012) and in the AB group, the statistical difference was close to the level of significance (p=0.052). Therefore, given the resluts, it is clear that the training of the inspiratory muscles should be an important part of the comprehensive therapy in patients with chronic lung disease.
