Physiotherapy in patients with chronic obstructive pulmonary disease

Abstract:

Název: Fyzioterapie u pacienta s chronickou obstrukční plicní nemocí Cíl: Cílem práce je přispět k řešení problematiky volby optimálních fyzioterapeutických postupů, konceptů a metod u vybraného klienta s chronickou obstrukční plicní nemocí. Metodika: Anamnestické údaje byly získávány řízeným rozhovorem. Testováním se měřil výkon pacientky v daných úkonech a pozorováním byly sledovány a následně popisovány reálné jevy a procesy, které není možné získat z pouhého rozhovoru, který je subjektivní. K testování byla zvolena goniometrie, antropometrie, testování rozvíjení páteře, brániční test a testy na hluboký stabilizační systém. Z fyzioterapeutických metod byla využita respirační fyzioterapie: dechové gymnastiky, drenážní metody (autogenní drenáž, flutter, vibrační masáže), myofasciální techniky oblasti hrudníku, hrudní páteře a ramenního kloubu, míčkovou facilitaci, mobilizace žeber, hrudní páteře a krční páteře. Pro pacientku byl na základě kineziologického rozboru vypracován rehabilitační plán na devět týdnů. V metodické části bylo popsáno vstupní vyšetření klienta, návrh krátkodobého plánu, výstupní vyšetření klienta, dlouhodobý fyzioterapeutický plán a shrnutí práce s klientem. Kazuistika klienta byla uvedena anonymně na základě podepsání souhlasu se zpracováním citlivých údajů. Ke zlepšení mobility hrudní páteře byly zvoleny mobilizace dle Lewita. Dynamická dechová gymnastiku byla zapojena pro udržení mobility hrudního koše. Pro uvolnění hypertonických svalů (m. pectoralis major, sternocleidomastoideus, serratus anterior, musculus trapezius a levator scapulae vlevo) byla využita postizometrická relaxace, myofasciální tecniky a míčková facilitace. Na základě výstupního vyšetření bylo zaznamenáno několik zlepšení: relaxace a zmírnění dušnosti při využití úlevových poloh, uvolnění fascií a hypertonických svalů v oblasti hrudníku pomocí myofasciálních technik a míčkování hrudníku, protažení měkkých tkání, podpoření expektorace sputa, zlepšení aktivace bránice nácvikem statické dechové gymnastiky, zlepšení pohyblivosti páteře pomocí mobilizace hrudní páteře dle Lewita, zlepšení pohyblivosti hrudníku a žeber mobilizačními technikami dle Lewita a snížení dechové frekvence o 4 dechy/min. i tepové frekvence o 16 tepů/min. pomocí statické dechové gymnastiky a nácviku bráničního dýchání.

Abstract:

Title: Physiotherapy of the patient with chronic obstructive pulmonary disease Goal: The goal of the thesis is to contribute to solving the problematics of choosing the optimal physiotherapic procedures, concepts and methods for particular patient with chronic obstructive pulmonary disease. Methodics: Directed interview was used to gain anamnestic data. The patient was tested and her performance during spesific tasks was recorded, followed by a description of real-life phenomenon not available through simple interview, since interview is subjective. For testing, it was chosen to use goniometry, anthropometry, spine examination, diaphragm test and testing of deep stabilizing muscles. There were several physiotherapy methods used respiratory physiotherapy, respiratory gymnastics, drainage methods (authogenic drainage, flutter, vibration massage), myofascial techniques of thorax, thoracic spine and shoulder joint, balling facilitation, rib, thoracic spine and cervical spine. Based on the kinosiologic analysis a 9-week rehabilitation plan was created. In methodical section is described the client examination, long-term physiotherapistic plan and the summary of my work with the client. Casuistry of the client was set anonymous based on the agreement about working with personal data. For enhancing the mobility of thoracic spine the mobilization according to Lewit was chosen. For keeping the chest mobility the dynamic gymnastics was used. For relaxing hypertonic muscles (m. pectoralis major, sternocleidomastoideus, serratus anterior, musculus trapezius and levator scapulae) a postisomatric relaxation, myofascial techniques and balling facilitation were used. Based of a final examination, several improvement were noticed: relaxation and stuffiness mitigation during relief positions, relaxation of fascia and hypertonic chest muscles using myofascial techniques and balling of the chest, stretching of soft tissues, supporting of shackle expectoration, improvement of diaphragm activation using rehearsal of static respiratory gymnastics, improving of spinal mobility using the thoracic spine mobilization according Lewit and lowering the respiratory rate by 4 breaths per minute, as well as heart rate by 16 pulses per minute using the static respiratory gymnastics and diaphragmic breathing rehearsal.
