Identification of cancer stem cells in selected childhood tumors

Abstract:

Disertační práce se zabývá podrobným studiem exprese nestinu a CD133 v high-grade osteosarkomech a rabdomyosarkomech. Imunohistochemicky byl vyšetřen materiál získaný od 18 pacientů s high-grade osteosarkomy a ve všech vzorcích byly detekovány nestin+ nádorové buňky. Exprese nestinu byla studována také na 4 buněčných liniích derivovaných z příslušných primárních tumorů. V celkem 3 zkoumaných liniích byly detekovány zřetelně nestin+ buňky. Ve 3 buněčných liniích byly také identifikovány buňky koexprimující nestin a CD133. Prezentované výsledky představují první důkaz exprese nestinu v osteosarkomech a zároveň v těchto tumorech ukazují na možnou přítomnost buněk s fenotypem nádorových kmenových buněk. Analýza exprese nestinu a CD133 byla také provedena na 10 vzorcích rabdomyosarkomů a 5 rabdomyosarkomových buněčných liniích. Oba markery byly pomocí imunohistochemie a imunofluorescence detekovány ve všech tkáních rabdomyosarkomů i ve všech buněčných liniích; v rabdomyosarkomových liniích byly také identifikovány sporadické CD133+/nestin+ buňky. Ve všech zkoumaných buněčných liniích byla prokázána exprese Oct3/4 a nucleosteminu, tedy proteinů zodpovědných za zachování unikátních vlastností kmenových buněk. Klonogenní a tumorigenní potenciál NSTS-11 buněk byl navíc prokázán pomocí funkčních experimentů: in vitro byly NSTS-11 buňky schopné tvořit kolonie; v in vivo experimentu byla na třech NOD/SCID myších ověřena schopnost těchto buněk tvořit xenograftové tumory s imunohistochemicky potvrzenou myogenní diferenciací. Lze shrnout, že tyto výsledky představují první důkazy o CD133 expresi v rabdomyosarkomech a korespondujících buněčných liniích. Koexprese CD133 a nestinu spolu s výsledky funkčních testů naznačují, že v rabdomyosarkomech jsou přítomny nádorové buňky s fenotypem nádorových kmenových buněk. U souboru 45 pacientů s high-grade osteosarkomy byl hodnocen prognostický význam exprese nestinu. Vysoká úroveň exprese nestinu detekovaná imunofluorescenčně byla spojena se signifikantně horší prognózou. Při užití imunohistochemie byly zaznamenány pouze trendy ke kratšímu přežití, výsledky však nedosáhly statistické významnosti. Za senzitivnější metodu aplikovanou na formol-parafínový materiál je považována imunohistochemie. Na základě našich výsledků je tudíž využití nestinu jako silného a nezávislého prognostického markeru pro pacienty s high-grade osteosarkomy limitované. Studium biologie nádorových kmenových buněk a jejich mikroprostředí patří bezesporu mezi nejprogresivnější oblasti lékařského výzkumu, jehož cílem je zvýšit efektivitu onkologické léčby pomocí terapie zacílené proti nádorovýcm kmenovým buňkám. Zjištění prezentovaná v této práci představují první krok směřující k spolehlivé identifikaci subpopulace nádorových buněk s fenotypem nádorových kmenových buněk v rabdomyosarkomech a high-grade osteosarkomech.

Abstract:

This thesis is primary focused on detailed study of nestin and CD133 expression in high-grade osteosarcomas and rhabdomyosarcomas. Using immunohistochemistry, nestin+ tumor cells were demonstrated in all tissue samples taken from 18 patients with high-grade osteosarcomas. Immunodetection of nestin was performed on 4 osteosarcoma-derived cell lines as well. Three of the established osteosarcoma cell lines were clearly nestin+. Furthermore, the co-expression of nestin and CD133 was detected in 3 cell lines. These results represent the first evidence of nestin expression in osteosarcomas and suggest the possible presence of cancer cells with a stem-like phenotype in these tumors. Analysis of nestin and CD133 expression was also performed in 10 tumor tissue samples taken from 7 patients with rhabdomyosarcomas and 5 corresponding cell lines. Using immunohistochemistry and immunofluorescence, both markers were detected in all tissue samples as well as in all cell lines. In rhabdomyosarcoma cell lines, sporadic CD133+/nestin+ cells were identified. All of derived cell lines expressed Oct3/4 and nucleostemin – proteins, which are required for maintaining stemness. Moreover, clonogenic and tumorigenic potential of NSTS-11 cells was demonstrated by functional assays: the ability to self-renew was confirmed in vitro using single cell colony forming assay; NSTS-11 cells formed xenograft tumors in NOD/SCID mice and myogenic differentiation of xenograft tumors was confirmed by immunohistochemistry. To summarize, these results represent the first evidence of CD133 expression in rhabdomyosarcomas and corresponding cell lines. Moreover, the co-expression of CD133 and nestin as well as results of the functional assays suggest a possible presence of cancer cells with phenotype of cancer stem cells in rhabdomyosarcomas. The prognostic value of nestin expression was assessed in the study cohort consisted of 45 patients with high-grade osteosarcomas. High levels of nestin expression detected by immunofluorescence were associated with significantly worse clinical outcomes. Using immunohistochemistry, trends toward shorter overall survival (OS) and event-free survival (EFS) were revealed, however, these differences did not reach statistical significance. When formalin-fixed and paraffin-embedded material is used, immunohistochemistry is regarded as a more sensitive method. Considering the above mentioned information, the use of nestin as a strong and independent prognostic marker for patients with high-grade osteosarcomas is limited. The study of cancer stem cells biology is one of the most progressive medical research aimed at improving the efficiency of cancer treatment using anti-cancer stem cells targeted therapy. The data presented in this thesis represent the first step towards reliable identification of subpopulations of tumor cells with cancer stemc ells phenotype in rhabdomyosarcomas and high-grade osteosarcomas
