History of nursing care for the mentally ill in today's Czech Republic

Abstract:

Cílem diplomové práce bylo prozkoumat historický vývoj ošetřovatelské péče o duševně nemocné, vzdělávání ošetřovatelů, pracovní náplň ošetřovatelů, jejich povinnosti, ale i další oblasti, které nám mohly objasnit, jak probíhala péče o choromyslné v prvních ústavech o duševně nemocné založených na území nynější České republiky. Využita byla metoda sondy, která nám na základě několika vybraných ústavů zobecnila vyhledané informace pro všechny tehdejší ústavy na území ČR. Potřebné materiály byly získávány z Moravského zemského archivu v Brně, z Národní lékařské knihovny v Praze, z Vědecké knihovny v Olomouci a z knihoven psychiatrických nemocnic. Po probádání všech hlavních dokumentů jsme došli k závěru, že péče o choromyslné, práce ošetřovatelů i jejich přístup nesl stejné znaky ve všech prostudovaných ústavech, z nichž ty nejdůležitější nyní popíšeme. Byl kladen důraz na laskavé jednání, trpělivost, vstřícnost, ohleduplnost vůči nemocným, na které museli ošetřovatelé neustále dohlížet a kontrolovat je, kvůli možnému uschovávání nebezpečných předmětů. Omezovací prostředky se mohly používat pouze v nejkrajnějších případech. Ošetřovatelé nemocným podávali léky a aplikovali léčebné procedury. V udržování hygieny a čistoty nemocných jim byli ošetřovatelé nápomocní, anebo za ně tyto úkony prováděli sami, pokud toho nebyli schopni. Ke vzdělávání ošetřovatelů sloužila školení a kurzy vedené sekundárními lékaři, ovšem nelze s jistotou tvrdit, že se vykonávaly ve všech ústavech. V pozdějším období posloužila ke vzdělávání učebnice od MUDr. Jana Hrašeho. Lze tedy zhodnotit, že péče o duševně nemocné byla již na počátcích ústavního léčení (po vzniku samostatných ústavů) velice pokroková a některé principy ošetřovatelské péče o duševně nemocné se s menšími obměnami používají dodnes.

Abstract:

The aim of this thesis was to investigate the historical development of nursing care for the mentally ill, the education of nurses, the job description of nurses, their duties, etc. Another goal was to clarify how the care of the mentally ill was provided in the first institutions for the mentally ill established in the Czech Republic. It used the probe method which generalized the information for selected institutions in the Czech Republic. The necessary materials were obtained from the Moravian Provincial Archives in Brno, the National Medical Library in Prague, the Scientific Library in Olomouc, and the libraries of psychiatric hospitals. After the analysis of all the main documents, it was concluded that the care of the mentally ill people, the work of the nurses, and their approach had the same characteristics in all described institutions studied. The most important are described in this thesis. Carers were working with kindness, patience, helpfulness, and respectfulness to the ill patients who had to be constantly supervised and checked by them because of the possible storage of dangerous objects. Restraints could only be used in the most extreme cases. Nurses administered medication and applied medical treatments to patients. The nurses assisted the patients in maintaining their hygiene and cleanliness or performed these tasks themselves if they were unable to do so. Training and courses run by secondary physicians were used to educate nurses, but it cannot be said with certainty that they were carried out in all institutions. In the later period, a textbook by Jan Hraše, M.D., was used for training. It can be assessed that the care of the mentally ill was already very advanced in the early days of institutional treatment (after the establishment of independent institutions), and some of the principles of nursing care for the mentally ill are still used today with minor variations.
