Setkání dvou depresí: Zkušenost psychoterapeutek potýkajících se s depresí v terapii depresivních klientů

When depression meets depression: depressive female psychotherapists' experience with depressive clients.

Abstract:

Deprese si díky své vysoké prevalenci a značnému utrpení, které způsobuje postiženým, ale také celé společnosti, žádá značnou výzkumnou pozornost. Léčba deprese probíhá často skrze psychoterapii, sami terapeuti však před touto nemocí nejsou imunní. Výzkum se zabývá prožíváním psychoterapeutů, kteří přicházejí do kontaktu s depresivními klienty a přitom sami depresí trpí. Protože deprese a terapie depresivních klientů je prožívaná muži a ženami odlišně, zabývá se výzkum pouze zkušeností psychoterapeutek. Cílem je nalézt, jaká témata jsou v této zkušenosti pro psychoterapeutky významná a zjistit, zda se v této zkušenosti objevuje posilování vlastní depresivity tak, jak naznačují výzkumy nakažlivosti deprese. Byly provedeny retrospektivní rozhovory s pěti psychoterapeutkami, které v minulosti trpěly depresí a léčily v té době depresivní klienty. Rozhovory byly analyzovány s pomocí interpretativní fenomenologické analýzy a byly nalezeny tři kategorie témat: emoční prožívání bezprostředního kontaktu s depresivním klientem; vlastní deprese v kontextu psychoterapeutického vztahu a procesu; působení terapie depresivního klienta na vlastní depresi. Zvlášť významná témata, která prostupovala také ostatními tématy se ukázala především vyčerpávání se v terapii depresivních klientů, překračování zdravé empatie až k identifikaci s klientem, nalézání vlastní snížené sebehodnoty v terapii depresivních klientů a silná sebekontrola v terapii. Posilování vlastní deprese v kontaktu s depresivním klientem se ve zkušenosti psychoterapeutek skutečně objevovalo, ale pouze skrze posilování symptomů deprese, nikoliv skrze subjektivní zážitek vlastního posílení deprese. Respondentky si tohoto posílení vědomy nebyly. Výsledky studie by měly sloužit především supervizorům a psychoterapeutkám trpícím depresí k lepšímu pochopení vzájemnému setkání dvou depresí v kontextu psychoteraepie.

Abstract:

Depression deserves the highest attention of research due to its high prevalence among population and the serious suffering it causes to both an individual and society. Psychotherapy is an important tool for a depression treatment, however, psychotherapists themselves are not immune against this illness. This research deals with the psychotherapists' experience of treating depressive patients while suffering from depression. Since the ways in which depression is experienced are gender specific, and so is the experience of treating a depressive patient, the study focuses only on female psychotherapists. The aim of this research is to explore themes important to the female psychotherapists in this particular experience and to discover whether their own depression is somehow intensified by their patients’ depression, as the contagiousness depression studies commonly suggest. Retrospective interviews with five previously depressed female psychotherapists, who had treated depressive clients at the time of their illness, were conducted and then analysed using interpretative phenomenological analysis. Three main categories of themes emerged: emotional experience of direct contact with a depressive client; own depression within the context of psychotherapeutic relationship and process; and depressive client psychotherapy’s impact on own depression. The most significant themes, penetrating the majority of other themes, were mainly self-exhausting while working on depressive clients psychotherapy, crossing the boundaries between healthy empathy with the depressive clients and leaning towards self-identification with them, exploration of own decreased self-esteem, and intense self-control in depressive clients therapy. The signs of own depression’s intensification during and after contact with the depressive clients did emerge in the female psychotherapists’ testimonies, yet without their conscious notion of it, as it was rather the depressive symptoms which were intensified than the subjective experience of own depression. The research findings shall help female psychotherapists suffering from depression, as well as to their supervisors, in gaining a better understanding of what may happen when depression meets with another depression in the context of psychotherapy.
