The Relationship Between Actual and Preferred Communication Style of a Medical Doctor and Quality of Working Alliance in Patients with Medically Unexplained Symptoms

Abstract:

Cílem této studie bylo ověřit souvislost mezi komunikačním stylem lékaře a kvalitou pracovní aliance u pacientů s medicínsky nevysvětlenými symptomy a u běžných pacientů, kteří sloužili jako kontrolní skupina. Kromě tohoto hlavního vztahu byl zkoumán také vliv moderátorů – reaktance a ke zdraví vztaženému locus of control. Vzorek byl složen z 522 respondentů (302 běžných pacientů a 203 pacientů s medicínsky nevysvětlenými symptomy). Data byla sbírána pomocí standardizovaných dotazníků na reaktanci, psychosomatické symptomy (SOMS-2), aktuální a preferovaný komunikační styl lékaře, ke zdraví vztažený locus of control a kvalitu aliance mezi lékařem a pacientem. Výsledky ukázaly, že empatický komunikační styl predikuje kvalitu pracovní aliance mezi lékařem a pacientem a že zároveň souvisí i s její vyšší kvalitou. Dále z výsledků vyplývá, že pacienti s medicínsky nevysvětlenými symptomy hodnotí pracovní alianci mezi nimi a lékařem hůře než pacienti bez těchto symptomů. Analýza výsledků taktéž podpořila hypotézu, že mezi reaktancí a interním locus of control bude existovat signifikantní pozitivní vztah. Míra vnímaných obtíží pramenících z diagnostikovaného onemocnění má vliv na kvalitu aliance mezi pacientem a lékařem. Předpoklad, že pacienti s medicínsky nevysvětlenými symptomy mají ve větší míře vysoké hodnoty reaktance než běžní pacienti, však výsledky této studie nepodpořily. Tyto výsledky a jejich možné implikace do praxe jsou diskutovány v kontextu předešlých empirických zjištění.

Abstract:

The aim of this study was to test the relationship between actual and preferred communication style of a medical doctor and quality of working alliance in patients with medically unexplained symptoms. These patients served as a control group. Beside this main relationship, the effect of moderators was examined – reactance and health locus of control. The sample consisted of 522 respondents (302 common patients and 203 patients with medically unexplained symptoms). The data were collected through standardized questionnaires for reactance, psychosomatic symptoms (SOMS-2), actual and preferred communication style of a medical doctor, health locus of control and the quality of working alliance between a medical doctor and a patient. Results show that emphatic communication style predicts quality of working alliance between medical doctor and patient and is also associated with its higher quality. Another result of this research is that patients with medically unexplained symptoms evaluate the working alliance between them and the doctor worse than patients without these symptoms. The result analysis also supported the hypothesis that there is a significant positive relationship between reactance and internal health locus of control. Amount of perceived troubles originating from a diagnosed disease has an influence on the quality of alliance between doctor and patient. A premise that patients with medically unexplained symptoms tend to have higher scores in reactance than common patients was not proved. These results and their possible implications to practice are discussed in the context of previous empirical findings.
