Abstract:

Cílem této bakalářské práce je zpracováni teoretických a praktických znalostí týkajících se diagnózy. Subtrochanterická zlomenina stehenní kosti a jejich následného využití při podrobné kazuistice u pacientů s touto diagnózou. Metody: Bakalářská práce je rozdělena na část obecnou, speciální a kazuistiku. V obecné části je popsána anatomie kyčelního kloubu včetně popisu kloubního pouzdra, vazů a svalů, klasifikace zlomenin a základní principy léčby, včetně druhy osteosyntéz. V práci je také uvedena obecná struktura kosti a popis růstu a remodelace. Speciální část obsahuje obecné rehabilitační postupy, základní principy ergoterapii, sociální a psychické komplikace v souvislosti s subtrochanterickou zlomen-inou. Kazuistika pacienta po subtrochanterické zlomenině femuru zahrnuje anam-nézu, vstupní a výstupní kineziologický rozboru, cíle terapie, vytvoření krátkodobého a dlouhodobého plánu terapie, zhodnocení průběhu terapií a jejího efektu. Kazuistika byla zpracována během souvislé odborné praxe ve Fakultní nemocnici u svaté Anny v Brně na Doléčovacím a rehabilitačním oddělení. Výsledky: Podařilo se splnit cíle stanovené v krátkodobém terapeutickém plánu. U pacienta došlo ke zvýšení rozsahů pohybu v operovaném kyčelním kloubu, posílení oslabených svalů, uvolnění napětí ve svalech, protažení svalů zkrácených, zlepšení stereotypu chůze a ke zvýšení stability trupu ve stoji a při chůzi.

Abstract:

This bachelor thesis aims to process theoretical and practical knowledge related to the diagnosis. Subtrochanteric fracture of the femur and subsequent use in a detailed case report in patients with this diagnosis. Methods: The bachelor thesis is divided into general, special, and case studies. The general part describes the hip joint's anatomy, including a description of the joint capsule, ligaments, muscles, classification of fractures, and primary treatment principles, including osteosynthesis types. The work also presents the general structure of bone and a description of growth and remodelling. The special part contains general rehabilitation procedures, basic occupational therapy principles, social and psychological complications connected with a subtrochanteric fracture. After a subtrochanteric femoral fracture, the patient's case report includes anamnesis, input, and kinesiological output analysis, therapy goals, the creation of short-term and long-term therapy plans, and evaluation of the course of treatments and their effect. The case report was prepared during continuous profes-sional practice at the University Hospital at St. Anne's in Brno in the Aftercare and Rehabilitation Department. Results: The goals set in the short-term treatment plan were met. In the patient, there was an increase in the range of motion in the operated hip joint, strengthening of the weakened muscles, a release of muscle tension, stretching of the shortened muscles, an improvement in gait stereotype.
