Dopad genetických aberací u pacientů s chronickou lymfocytární leukémií léčených režimy zahrnujícími rituximab

Impact of genetic aberrations in patients with chronic lymphocytic leukemia treated with regimens including rituximab

Abstract:

Pro pacienty s chronickou lymfocytární leukémií (CLL) jsou režimy zahrnující rituximab v kombinaci s chemoterapií významnou terapeutickou možností. Dosavadní klinické studie prokázaly, že pacienti s mutací v genu TP53 z těchto režimů neprofitují a měli by být léčeni pomocí alternativních přístupů. Potenciální dopad dalších prognosticky důležitých mutací u pacientů s CLL zůstává nejasný, včetně jejich role v relapsu onemocnění. Náplní disertační práce byla analýza dopadu čtyř rekurentně mutovaných genů (ATM, SF3B1, NOTCH1, BIRC3), cytogenetických aberací (delece 11q22, delece 13q14, trisomie 12) a statutu IGHV na dobu do progrese onemocnění (PFS) u pacientů s CLL s intaktním genem TP53 léčených režimy FCR (fludarabin, cyklofosfamid, rituximab) nebo BR (bendamustin, rituximab) v 1. linii. Vedle toho byla rovněž prováděna analýza klonální evoluce mutací v relapsu. Celkově bylo analyzováno 120 pacientů léčených režimem FCR a 57 pacientů léčených režimem BR ve Fakultní nemocnici Brno. V univariačních analýzách byly pozorovány následující výstupy: (a) negativní dopad mutací v genu BIRC3 na PFS v obou léčebných režimech; (b) žádný dopad mutací v genech ATM, SF3B1 a NOTCH1 na PFS v obou léčebných režimech; (c) negativní dopad nemutovaného statutu IGHV na PFS v kohortě léčené FCR; (d) žádný dopad nemutovaného statutu IGHV na PFS v kohortě léčené BR; (e) negativní dopad delece 11q22 na PFS v obou léčebných režimech; (f) žádný dopad trisomie 12 a delece 13q14 na PFS v obou léčebných režimech. V multivariační analýze udržela nezávislou asociaci s redukovaným PFS pouze delece 11q22 v obou režimech a nemutovaný status IGHV v kohortě FCR. Analýza klonální evoluce vzorků v relapsu prokázala (a) u genu ATM čtyři expanze a dvě redukce procentuálního zastoupení mutací; (b) u genu SF3B1 jednu expanzi a dvě redukce; (c) žádnou změnu stavu mutace NOTCH1; (d) u genu BIRC3 dvě expanze a dvě redukce a (d) expanzi tří mutací v genu TP53. Celkově naše data ukazují, že u pacientů léčených chemoimunoterapií v rutinním klinickém prostředí může být většina relevantních prognostických informací získána analýzou pouze delece 11q22 a stavu IGHV u CLL pacientů s intaktním genem TP53. Klonální evoluce ukazuje v relapsu částečně odlišný mutační profil.

Abstract:

Regimens involving rituximab in combination with chemotherapy represent important therapeutic option for patients with chronic lymphocytic leukemia (CLL). Previous clinical studies have shown that patients with TP53 mutation do not benefit from these regimens and should be treated with alternative approaches. The potential impact of other prognostically important mutations in CLL patients remains unclear, including their role in a disease relapse. The aim of this thesis was to analyze the impact of four recurrently mutated genes (ATM, SF3B1, NOTCH1, BIRC3), cytogenetic aberrations (deletion 11q22, deletion 13q14, trisomy 12) and IGHV status on progression-free survival (PFS) in patients with CLL with an intact TP53 gene after front-line therapy with FCR (fludarabine, cyclophosphamide, rituximab) or BR (bendamustine, rituximab) regimens. In addition, the analysis of clonal evolution of mutations in relapse was also performed. Altogether, 120 patients were treated with FCR and 57 patients treated with BR at the University Hospital Brno. The following outcomes were observed in univariate analyzes: (a) negative impact of BIRC3 mutations on PFS in both regimens; (b) no effect of ATM, SF3B1 and NOTCH1 mutations on PFS in both regimens; (c) negative impact of unmutated IGHV status on PFS in the cohort treated with FCR; (d) no impact of unmutated IGHV status on PFS in the cohort treated with BR; (e) negative impact of deletion 11q22 on PFS in both regimens; (f) no effect of trisomy 12 and deletion 13q14 on PFS in both regimens. In a multivariate analysis, only deletion 11q22 in both regimens and unmutated IGHV in the FCR cohort maintained an independent association with the reduced PFS. The analysis of clonal evolution in relapse showed (a) four expansions and two reductions of mutations in ATM gene (b) one expansion and two reductions in SF3B1 gene; (c) no change of the status of the NOTCH1 mutations; (d) two expansions and two reductions in BIRC3 gene and (d) three expansion of the TP53 mutations. Overall, our data show that the most relevant prognostic information may be obtained by analyzing only deletion 11q22 and IGHV status in CLL patients with an intact TP53 gene treated with chemoimmunotherapy in routine clinical practise. The clonal evolution shows a partially different mutation profile in relapse.
