STEPS - group therapy-educational program for siblings of children with autism spectrum disorders: creation, implementation and evaluation of the program

Abstract:

Zdraví sourozenci dětí s poruchami autistického spektra (PAS) čelí řadě jedinečných psychologických a sociálních stresorů souvisejících s obtížemi jejich sourozence a jsou tak vystaveni zvýšenému riziku rozvoje psychických a adaptačních obtíží. V České republice byla této skupině dětí věnována doposud pouze minimální pozornost. Cílem dizertační práce proto bylo vytvořit, implementovat a výzkumně zhodnotit efektivitu podpůrného programu pro sourozence dětí s PAS - program STEPS (Skupinový Terapeuticko-Edukační Program pro Sourozence dětí s PAS). STEPS vychází zejména z teorie kognitivně-behaviorální terapie a zážitkové pedagogiky. Sestává z 10 dvouhodinových setkání pro dvě věkové skupiny (7–11 a 12–16 let). Hlavními cíli programu je posilování pozitivních aspektů sourozeneckého vztahu, předání vědomostí o PAS, zlepšení úrovně psychického přizpůsobení, kvality života, subjektivně vnímané rodinné opory, řešení problémů a copingových strategií. Finální verzí programu prošlo prozatím 31 sourozenců dětí s PAS a jejich rodičů. Pro ověření efektivity programu ve výše zmíněných cílových oblastech programu jsme užili smíšený výzkumný design a metodu opakovaných měření, která umožnila porovnání změn v období bez intervence, bezprostředně po intervenci a 3 měsíce po intervenci. Výsledky výzkumu naznačují, že absolvování programu STEPS souvisí s pozitivními změnami v sourozeneckém vztahu, navýšením kvality života v oblasti psychického fungování, adaptivnějším zvládáním problematických situací spojených se sourozencem, nárůstem subjektivně vnímané opory ze strany rodiny a výrazným navýšením vědomostí o PAS. Změny ve výše zmíněných doménách pravděpodobně nesouvisejí s pouhým zráním dítěte, ale s absolvováním programu STEPS. Ve všech oblastech, s výjimkou sourozeneckého vztahu, se úroveň dosažených změn stabilně udržuje i po 3 měsících od intervence. Účastníci i rodiče program pozitivně oceňovali a hodnotili jako přínosnou a pro ně relevantní formu podpory. Výše zmíněné výsledky společně s pozitivním hodnocením jak ze strany účastníků, tak jejich rodičů, svědčí o tom, že program STEPS může být přínosným a prakticky dobře aplikovatelným nástrojem podpory zdravých sourozenců dětí s PAS.

Abstract:

Healthy siblings of children with Autism Spectrum Disorders (ASD) are challenged with number of specific psychological and social stressors related to their siblings’ difficulties, that put them in an increased risk of developing mental health problems and adaptation difficulties. Yet this group has received only minor attention from practitioners and researchers in the Czech Republic. Therefore, the aim of this dissertation thesis was to create, implement and evaluate the STEPS intervention (Group Therapeutic and Educational Program for Siblings of Children with ASD). STEPS is primarily based on cognitive-behavioral therapy and experiential learning. It consists of 10 two-hour sessions designed for two age groups (7-11 and 12-16 years). The main objectives of the program are to strengthen the positive aspects of sibling relationship; to improve ASD knowledge; to increase the level of psychological adaptation, quality of life and subjectively perceived family support; and to strengthen adaptive problem solving skills and coping strategies. Thirty-one siblings and their parents have already taken part the intervention program. To evaluate the effectiveness of the program in the above-mentioned target areas, mixed research design has been used. Repeated measures allowed to compare changes in the intervention-free period, immediately after the intervention and 3 months after the intervention. The research results suggest that STEPS intervention is related to positive changes in sibling relationship, increased quality of life in the domain of psychological functioning, more adaptive coping with problematic situations associated with the sibling, increase of subjectively perceived family support, and an increase in ASD knowledge. Changes in the above-mentioned domains are unlikely to be related to the mere maturation, but correlate with the STEPS intervention. In all measured domains, except the sibling relationship, the level of improvement did not significantly change 3 months after the intervention. Participants and parents positively appreciated the intervention and conceived the program as a beneficial and relevant form of support. The results above together with a positive evaluation from both the participants and their parents suggest that STEPS intervention could be a beneficial and practically well–applicable tool for supporting healthy siblings of children with ASD.
